摘要
目的 通过数据挖掘技术探讨针灸治疗脑卒中后失语症的取穴规律.方法 选取国家知识基础设施数据库近5年关于针灸治疗脑卒中后语言障碍的文献,根据纳入与排除标准筛选合适文献,并通过SPSS Modeler 18.0、SPSS 25.0、Cytoscape及Origin 2023等软件对针灸处方进行可视化分析,主要内容包括穴位或选区出现频次、特定穴、归经、部位、关联规则及系统聚类分析等.结果 共纳入47篇文献,涉及腧穴103个、特定穴12类、经络14条.其中,频次最高的腧穴为廉泉、百会、玉液、金津、哑门、风池、水沟、风府等;特定穴以交会穴、八脉交会穴、合穴、输穴等为主;主要归经为督脉、足少阳胆经等;关联规则分析发现,金津-玉液、玉液-金津、水沟-百会、金津-廉泉-玉液、玉液-廉泉-金津、玉液-百会-金津等组合置信度、支持度最高;系统聚类分析可知,103个腧穴大致可以分为6大聚类.结论 针灸治疗脑卒中后失语症的取穴部位以头颈部、口腔等"近治"穴位为主,治疗选穴以督脉、足少阳胆经更为多见,特定穴以交会穴、八脉交会穴为主.
Abstract
Objective To explore the acupoint selection rule of acupuncture and moxibustion treatment of post-stroke aphasia by data mining technology.Methods The literatures on acupuncture and moxibustion treatment of post-stroke language disorder in the recent 5 years were selected from the National Knowledge Infrastructure database,suitable literatures were selected according to inclusion and exclusion criteria,and the acupuncture and moxibustion prescriptions were visually analyzed by SPSS Modeler 18.0,SPSS 25.0,Cytoscape and Origin 2023 software.The main contents include the frequency of occurrence of acupoints or constituencies,specific acupoints,meridian tropism,location,association rules and systematic cluster analysis.Results A total of 47 literatures were included,involving 103 acupoints,12 types of specific acupoints and 14 meridians.Among them,the acupoints with the highest frequency are Lianquan,Baihui,Yuye,Jinjin,Yamen,Fengchi,Shuigou,Fengfu,etc.The specific acupoints are mainly intersection points,eight-pulse intersection points,combined points and transfusion points.The main meridians are Du pulse,foot Shaoyang gallbladder meridian,etc.Association rule analysis found that Jinjin-Yuye,Yuye-Jinjin,Shuigou-Baihui,Jinjin-Lianquan-Yuye,Yuye-Lianquan-Jinjin,Yuye-Baihui-Jinjin and other combinations had the highest confidence and support.Systematic cluster analysis shows that 103 acupoints can be roughly divided into 6 clusters.Conclusion The acupoints of acupuncture and moxibustion for post-stroke aphasia are mainly head and neck,oral cavity and other'near treatment'acupoints.The treatment acupoints are more common in the Du pulse and foot Shaoyang gallbladder meridian.The specific acupoints are mainly the intersection points and the eight-pulse intersection points.
