摘要
目的 探讨宣痹通脉汤对急性心肌梗死患者心功能的影响.方法 将2022年1月～2022年12月于我院收治的68例急性心肌梗死患者按照随机数字表法分为对照组及观察组.对照组34例进行常规西药治疗,观察组34例在西药治疗基础上进行宣痹通脉汤治疗.比较两组患者临床疗效,治疗前后氨基末端利钠肽前体(NT-proBNP)、心肌肌钙蛋白I(cTnI)水平、心功能指标[左心室收缩末期容积(LVESV)、左心室舒张末期容积(LVEDV)、心脏指数(CI)]及中医证候积分变化情况.结果 治疗前,对照组和观察组的心功能指标水平、NT-proBNP、cTnI水平和中医证候积分比较,差异均无统计学意义(P＞0.05);治疗后与同组治疗前相比,两组NT-proBNP、cTnI、LVESV、LVEDV水平及中医证候积分均明显降低,且观察组明显更低(P＜0.05).治疗后,两组CI均明显升高,且与对照组相比,观察组明显更高(P＜0.05).对照组的临床有效率为70.59%,观察组的临床有效率为91.18%,与对照组相比,观察组明显更高(P＜0.05).结论 宣痹通脉汤治疗可显著改善急性心肌梗死患者的心室重构,提高患者心功能,进而提高治疗效果.
Abstract
Objective To investigate the effects of Xuanbi Tongmai Decoction on cardiac function in patients with acute myocardial infarction.Methods 68 patients with acute myocardial infarction admitted to our hospital from January 2022 to December 2022 were divided into control group and observation group according to random number table method.34 cases in control group received conventional western medicine treatment,34 cases in observation group received Xuanbi Tongmai decoction on the basis of western medicine treatment.The clinical efficacy,the levels of N terminal pro B type natriuretic peptide(NT-proBNP),cardiac troponin I(cTnI),cardiac function index[left ventricular end systolic volume(LVESV),left ventricular end diastolic volume(LVEDV),cardiac index(CI)]and traditional Chinese medicine(TCM)syndrome scores before and after treatment were compared between the two groups.Results Before treatment,there were no significant differences in the levels of cardiac function index,NT-proBNP,cTnI and TCM syndrome scores between the control group and the observation group(P＞0.05).After treatment,the levels of NT-proBNP,cTnI,LVESV,LVEDV and TCM syndrome scores of the two groups were significantly lower than those in the same group before treatment,and those in the observation group were significantly lower(P＜0.05).After treatment,the CI of both groups increased significantly,and the observation group was significantly higher than the control group(P＜0.05).The clinical effective rate of the control group was 70.59%,and that of the observation group was 91.18%,which was significantly higher than that of the control group(P＜0.05).Conclusion Xuanbi Tongmai Decoction can significantly improve the ventricular remodeling and cardiac function of patients with acute myocardial infarction,and thus improve the therapeutic effect.
