摘要

目的 通过对比观察癫痫合并心肌损伤患者和癫痫未合并心肌损伤患者外周血的细胞外信号调节激酶1/2(ERK1/2)的表达情况及高压氧干预的疗效,探讨ERKl/2用于评估癫痫合并心肌损伤的临床价值及高压氧的保护机制.方法 选择2020年1月~2022年12月临沂市中心医院神经内科收治的早期癫痫持续状态患者58例,根据肌酸激酶同工酶(CK-MB)水平分为单纯癫痫组27例,癫痫合并心肌损伤组31例,并选择25例同期健康体检者纳入对照组,单纯癫痫组给予常规抗癫痫治疗,癫痫合并心肌损伤组在常规治疗的基础上给予高压氧(HBO)治疗.比较三组的ERK1/2、CK-MB水平;并观察癫痫合并心肌损伤组患者经HBO干预前后ERK1/2、CK-MB水平的变化.结果 癫痫合并心肌损伤组患者ERK1/2水平明显低于单纯癫痫组及对照组,CK-MB明显高于单纯癫痫组及对照组,差异均有统计学意义(P<0.05).癫痫合并心肌损伤组患者经HBO干预后,CK-MB水平下降,ERK1/2水平升高,差异均有统计学意义(P<0.05).结论 早期癫痫持续状态合并心肌损伤患者ERK1/2水平降低,HBO对癫痫相关的心肌损伤有较好的治疗效果,升高ERKl/2水平可能是HBO治疗癫痫相关心肌损害的机制.

Abstract

Objective By comparing the expression of extracellular signal-regulated kinase 1/2(ERK1/2)in peripheral blood of patients with epilepsy combined with myocardial injury and epilepsy without myocardial injury,and the efficacy of hyperbaric oxygen intervention.The clinical value of ERK1/2 in evaluating epilepsy complicated with myocardial injury and the protective mechanism of hyperbaric oxygen were discussed.Methods 58 patients with early status epilepticus admitted to the Department of Neurology,Linyi Central Hospital from January 2020 to December 2022 were selected.According to the level of creatine kinase isoenzyme(CK-MB),the patients with epilepsy were divided into simple epilepsy group(27 cases)and epilepsy combined with myocardial injury group(31 cases),and 25 healthy subjects were selected as the control group.The simple epilepsy group was given conventional anti-epileptic treatment,and the epilepsy combined with myocardial injury group was given hyperbaric oxygen(HBO)treatment on the basis of conventional treatment.The levels of ERK1/2 and CK-MB in the three groups were compared.The changes of ERK1/2 and CK-MB levels in patients with epilepsy combined with myocardial injury before and after HBO intervention were observed.Results The level of ERK1/2 in the epilepsy combined with myocardial injury group was significantly lower than that in the simple epilepsy group and the control group,and the CK-MB was significantly higher than that in the simple epilepsy group and the control group,the differences were statistically significant(P<0.05).After HBO intervention,the level of CK-MB decreased and the level of ERK1/2 increased in the epilepsy combined with myocardial injury group,the differences were statistically significant(P<0.05).Conclusion The level of ERK1/2 is decreased in patients with early status epilepticus combined with myocardial injury.HBO has a good therapeutic effect on epilepsy-related myocardial injury.Elevated ERK1/2 level may be the mechanism of HBO in the treatment of epilepsy-related myocardial injury.
