摘要
目的 探究血液灌流和血液透析联合慢性病自我管理对慢性肾功能衰竭尿毒症患者肾功能的影响.方法 选取我院2021 年 1 月～2023 年 1 月符合条件的 84 例慢性肾功能衰竭尿毒症患者,按照抽签法分为对照组(n=42)和研究组(n=42).对照组采用血液透析联合慢性病自我管理进行治疗,研究组采用血液灌流和血液透析联合慢性病自我管理进行治疗,评估两组患者的肾功能指标、电解质指标、自我管理能力以及并发症发生情况.结果 治疗前两组尿红细胞沉降率(ESR)、尿蛋白(Upro)、血清尿素氮(BUN)、β2 微球蛋白(β2-MG)、钾离子(K+)、钠离子(Na+)、氯离子(Cl-)水平比较,差异均无统计学意义(P＞0.05),治疗后研究组尿ESR、Upro、BUN、β2-MG、K+、Cl-明显低于对照组,Na+明显高于对照组,差异有统计学意义(P＜0.05);治疗后研究组自我管理能力优于对照组,差异有统计学意义(P＜0.05);研究组感染、尿毒症脑病、皮肤瘙痒、心脏病变的总发生率明显低于对照组(P＜0.05).结论 血液灌流和血液透析联合慢性病自我管理在慢性肾功能衰竭尿毒症患者中的应用效果较好,可改善患者肾功能,调节电解质平衡,提高患者自我管理能力,减少并发症的发生.
Abstract
Objective To explore the effect of hemoperfusion and hemodialysis combined with chronic disease self-management on renal function in patients with chronic renal failure uremia.Methods 84 eligible patients with chronic renal failure uremia in our hospital from January 2021 to January 2023 were selected and divided into control group(n=42)and study group(n=42)according to lottery method.The control group was treated with hemodialysis combined with chronic disease self-management,while the study group was treated with hemoperfusion and hemodialysis combined with chronic disease self-management.The renal function indexes,electrolyte indexes,self-management ability and complications of the two groups were evaluated.Results Before treatment,there were no significant differences in urinary erythrocyte sedimentation rate(ESR),urinary protein(Upro),serum urea nitrogen(BUN),β2 microglobulin(β2-MG),potassium ion(K+),sodium ion(Na+)and chloride ion(Cl-)levels between two groups(P>0.05).After treatment,urinary ESR,Upro,BUN,β2-MG,K+,Cl-in the study group were significantly lower than those in the control group,and Na+ was significantly higher than that in the control group,the difference was statistically significant(P<0.05).After treatment,the self-management ability of the study group was better than that of the control group,the difference was statistically significant(P<0.05).The total incidence of infection,uremic encephalopathy,skin pruritus and heart disease in the study group was significantly lower than that in the control group(P<0.05).Conclusion The application effect of hemoperfusion and hemodialysis combined with chronic disease self-management in patients with chronic renal failure uremia is better,which can improve patients'renal function,regulate electrolyte balance,improve patients'self-management ability,and reduce the occurrence of complications.
