摘要

目的 探讨术前口服营养补充(ONS)对结直肠癌患者术后的营养状况和临床预后的影响.方法 回顾性收集我院2020年4月1日~2021年3月31日接受结直肠癌根治术的108例患者的资料.根据术前营养支持方式不同,51例术前予ONS的患者为观察组,57例术前予普通饮食的患者为对照组;在亚组分析中,根据患者使用的ONS制剂是否含ω-3多不饱和脂肪酸(ω-3 PUFAs)分为肠内营养乳剂(TPF-T)组和肠内营养粉剂(TP)组.对比评估术前ONS对患者术后营养状态、肝肾功能、临床预后的影响.结果 观察组术后第7天血清白蛋白水平高于对照组,差异有统计学意义(P<0.05);观察组术后住院时间少于对照组,差异有统计学意义(P<0.05);TPF-T组患者术后第7天CRP水平显著低于TP组,差异有统计学意义(P<0.05).结论 术前予ONS有利于改善结直肠癌根治术患者的术后营养状态,缩短住院时间,改善临床预后;术前补充ω-3 PUFAs,可减轻患者术后炎症反应.

Abstract

Objective To investigate the effects of preoperative oral nutritional supplementation(ONS)on postoperative nutritional status and clinical prognosis of patients with colorectal cancer.Methods The data of 108 patients who underwent gastrointestinal tumor resection in our hospital from April 1,2020 to March 31,2021 were retrospectively collected.According to the different ways of preoperative nutritional support,The patients were treated with preoperative ONS was classified as observation group(51 cases),and 57 patients with normal diet before surgery were classified as control group.In subgroup analysis,patients were divided into enteral nutritional emulsion(TPF-T)group and enteral nutritional powder(TP)group based on whether the ONS preparation used contained omega-3 polyunsaturated fatty acids(omega-3 PUFAs).The effects of preoperative ONS on postoperative nutritional status,liver and kidney function and clinical prognosis were compared and evaluated.Results The serum albumin level in the observation group was higher than that in the control group on the 7th day after operation,the difference was statistically significant(P<0.05);The postoperative hospitalization time in the observation group was significantly shorter than that in the control group,the difference was statistically significant(P<0.05);the C-reactive protein level in the TPF-T group was lower than that in the TP group on the 7th day after surgery,and the difference was statistically significant(P<0.05).Conclusion Preoperative ONS is beneficial to improve the nutritional status of patients after radical resection of colorectal cancer,shorten the length of hospital stay and improve the clinical prognosis.Preoperative supplementation of omega-3 PUFAs can reduce postoperative inflammation.
