摘要

目的 分析T细胞亚群及血清白细胞介素(IL)-6、IL-17水平对类风湿关节炎(RA)预后的影响.方法 选取2021年9月～2023年9月收治的102例RA患者为研究对象,均接受甲氨蝶呤片治疗3个月,于治疗前、治疗后采集患者空腹血液标本,使用流式细胞仪测定T细胞亚群(CD4+、CD8+)水平,使用酶联免疫吸附法(ELISA)测定血清IL-6、IL-17水平.统计所有患者临床预后情况,比较不同预后患者T细胞亚群及血清IL-6、IL-17水平.结果 RA患者重度活动组的CD4+/CD8+、血清IL-6、IL-17水平均高于中度及轻度活动组(P＜0.05).RA患者治疗后CD4+、血清IL-6、IL-17水平较治疗前降低,CD8+较治疗前升高(P＜0.05).102例RA患者中,72例预后良好,占比70.59%,30例为预后不良,占比29.41%.预后不良的RA患者治疗后CD4+、血清IL-6、IL-17水平较预后良好者高,CD8+较预后良好者低(P＜0.05).结论 CD4+、CD8+、CD4+/CD8+、血清IL-6、IL-17水平与RA患者预后情况显著相关.

Abstract

Objective To analyze the effects of T-cell subsets,serum interleukin(IL)-6 and IL-17 levels on the prognosis of rheumatoid arthritis(RA).Methods A total of 102 RA patients admitted from September 2021 to September 2023 were selected as the study subjects.All patients were treated with methotrexate tablets for 3 months.Fasting blood specimens were collected from the patients before and after treatment.The levels of T-cell subsets(CD4+,CD8+)were determined using flow cytometry,and the levels of serum IL-6 and IL-17 were determined using enzyme-linked immunosorbent assay(ELISA).The clinical prognosis of all patients was statistically analyzed,and the T-cell subsets and serum IL-6 and IL-17 levels of patients with different prognoses were compared.Results The levels of CD4+/CD8+,serum IL-6 and IL-17 in severe active group of RA patients were higher than those in moderate and mild active groups(P<0.05).CD4+,serum IL-6,IL-17 levels were lower and CD8+was higher in RA patients after treatment than those before treatment(P<0.05).Among the 102 RA patients,72 cases had a good prognosis,accounting for 70.59%,and 30 cases had a poor prognosis,accounting for 29.41%.CD4+,serum IL-6 and IL-17 levels after treatment were higher in poor prognosis RA patients than those in good prognosis ones,and CD8+was lower than that in good prognosis ones(P<0.05).Conclusion CD4+,CD8+,CD4+/CD8+,serum IL-6 and IL-17 levels are strongly associated with the prognosis of RA patients.
