摘要

目的 分析盐酸氟桂利嗪联合托吡酯治疗前庭性偏头痛的效果及对前庭功能的影响.方法 选择2022年1月～2023年9月在我院接受前庭性偏头痛治疗的患者80例进行回顾性分析,根据治疗方案不同分为对照组(治疗方案:盐酸氟桂利嗪)和观察组(治疗方案:盐酸氟桂利嗪联合托吡酯),每组40例,对比治疗后两组的总有效率、头痛与眩晕症状以及前庭功能改善效果差异.结果 观察组治疗后总有效率为95.00%,高于对照组的80.00%(P＜0.05);两组治疗前的头痛视觉模拟评分法(VAS)评分、眩晕障碍量表(DHI)评分、眩晕及头痛持续时间无明显差异(P＞0.05),治疗后,观察组头痛VAS评分、DHI评分低于对照组,且眩晕及头痛持续时间比对照组更短(P＜0.05);两组治疗前的视动性眼震异常、凝视诱发性眼震异常、自发性眼震阳性例数比较,无明显差异(P＞0.05),治疗后,两组视动性眼震异常、凝视诱发性眼震异常、自发性眼震阳性例数均较治疗前减少(P＜0.05);且观察组视动性眼震异常、凝视诱发性眼震异常、自发性眼震阳性例数低于对照组(P＜0.05).结论 盐酸氟桂利嗪联合托吡酯治疗前庭性偏头痛,有助于提升整体疗效,不仅能减轻患者的头痛与眩晕症状,还能改善前庭功能.

Abstract

Objective To analyze the effect of Flunarizine Hydrochloride combined with Topiramate in the treatment of vestibular migraine and its impact on vestibular function.Methods A total of 80 patients with vestibular migraine who were treated in our hospital from January 2022 to September 2023 were retrospectively analyzed.According to different treatment plans,they were divided into control group(treatment plan:flunarizine hydrochloride)and observation group(treatment plan:flunarizine hydrochloride combined with topiramate),with 40 patients in each group.The total effective rate,headache and dizziness symptoms,and vestibular function improvement effect were compared between the two groups after treatment.Results The total effective rate of the observation group after treatment was 95.00%,which was higher than that of the control group(80.00%)(P<0.05).There was no significant difference in headache visual analogue scale(VAS)score,dizziness handicap inventory(DHI)score,dizziness and headache duration between the two groups before treatment(P>0.05).After treatment,the headache VAS score and DHI score in the observation group were lower than those in the control group,and the duration of dizziness and headache was shorter than that in the control group(P<0.05).There was no significant difference in the number of patients with abnormal optokinetic nystagmus,abnormal gaze-induced nystagmus and positive spontaneous nystagmus between the two groups before treatment(P>0.05).After treatment,the number of patients with abnormal optokinetic nystagmus,abnormal gaze-induced nystagmus and positive spontaneous nystagmus in the two groups was lower than that before treatment(P<0.05).The number of patients with abnormal optokinetic nystagmus,abnormal gaze-induced nystagmus and positive spontaneous nystagmus in the observation group was lower than that in the control group(P<0.05).Conclusion Flunarizine hydrochloride combined with topiramate in the treatment of vestibular migraine is helpful to improve the overall efficacy,which can not only reduce the symptoms of headache and dizziness,but also improve vestibular function.
