甲基泼尼松龙联合免疫球蛋白治疗重症肺炎支原体肺炎的疗效及对免疫功能的影响

摘要
目的 研究甲基泼尼松龙联合免疫球蛋白治疗重症肺炎支原体肺炎(MPP)的疗效及对免疫功能的影响.方法 选取2022年12月~2023年12月我院收治的120例重症MPP患儿作为研究对象,按随机数字表法分为甲基泼尼松龙组和免疫球蛋白组,各60例.两组均予以常规治疗,甲基泼尼松龙组予以甲基泼尼松龙治疗,免疫球蛋白组在甲基泼尼松龙组治疗方案的基础上联合免疫球蛋白治疗,两组疗程均为7 d,观察两组的疗效以及患者在治疗过程中出现的不良反应,并比较两组治疗前后血清免疫球蛋白、血清降钙素原(PCT)、补体C3、C4水平.结果 治疗后,免疫球蛋白组总有效率高于甲基泼尼松龙组(P<0.05);与治疗前相比,两组治疗后血清免疫球蛋白水平均升高,血清PCT和补体C3、C4水平均下降,且免疫球蛋白组的血清免疫球蛋白水平升高幅度大于甲基泼尼松龙组,血清PCT和补体C3、C4水平下降幅度大于甲基泼尼松龙组(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05).结论 甲基泼尼松龙联合免疫球蛋白治疗能有效控制重症MPP患儿的病情,提高患儿体内免疫球蛋白水平,同时减轻补体过度激活引发的免疫损伤,进而改善患儿免疫功能,且安全性较好.
Abstract
Objective To study the effect of methylprednisolone combined with immunoglobulin in the treatment of severe Mycoplasma pneumoniae pneumonia(MPP)and its effect on immune function.Methods A total of 120 children with severe MPP admitted to our hospital from December 2022 to December 2023 were selected as the research objects.They were divided into methylprednisolone group and immunoglobulin group according to the random number table method,with 60 cases in each group.Both groups were given conventional treatment.The methylprednisolone group was treated with methylprednisolone,and the immunoglobulin group was treated with immunoglobulin on the basis of the treatment plan of the methylprednisolone group.Both groups were treated for 7 days.The efficacy of the two groups and the adverse reactions of the patients during the treatment were observed,and the levels of serum immunoglobulin,serum procalcitonin(PCT),complement C3 and C4 were compared between the two groups before and after treatment.Results After treatment,the total effective rate of the immunoglobulin group was higher than that of the methylprednisolone group(P<0.05).Compared with before treatment,the levels of serum immunoglobulin in the two groups increased after treatment,while the levels of serum PCT and complement C3 and C4 decreased.The increase of serum immunoglobulin level in the immunoglobulin group was greater than that in the methylprednisolone group,and the decrease of serum PCT and complement C3 and C4 levels was greater than that in the methylprednisolone group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Methylprednisolone combined with immunoglobulin therapy can effectively control the disease conditions of children with severe MPP,improve the level of immunoglobulin in children,reduce the immune injury caused by excessive activation of complement,and improve the immune function of children,with good safety.
