摘要

目的:研究成人安氏Ⅱ类1分类高角病例拔除4个第一前磨牙并分别应用微种植体支抗和传统支抗矫治后其牙合平面和下颌平面的变化.方法:选取诊断为安氏Ⅱ类1分类高角并通过拔除4个第一前磨牙矫治后的患者40例,年龄18~30岁;微种植钉支抗组(G1)和传统支抗组(G2)各20例;收集两组患者矫治前后的头颅侧位片并对其进行测量分析.结果:G1组矫治前后相比:G1组矫治前后,U1-PP压低了(0.62±0.62)mm(P<0.05),SNA角减小了(0.93±1.09)°(P<0.05).G2组矫治前后相比:U1-PP伸长(0.94±1.27)mm(P<0.05),U6-PP伸长了(1.41±1.00)mm(P<0.05),U6-SV近中移动了(2.56±1.46)mm(P<0.05),OP-SN角增加了(2.40±1.48)°(P<0.05),SNA角减小了(0.85±0.74)°(P<0.05),SNB角减小了(1.23±0.88)°(P<0.05),Y轴角增加了(1.20±1.36)°(P<0.05),下颌平面角(MP-SN、GoGn-SN)分别增加了(1.18±1.03)°、(1.00±0.99)°(P<0.05).其他测量项目治疗前后相比均无统计学差异(P>0.05).结论:成人安氏Ⅱ类1分类高角通过拔除4个第一前磨牙进行治疗的患者利用微种植钉支抗关闭拔牙间隙时,对颌平面及下颌平面的控制与传统支抗相比具有明显的优势.

Abstract

AIM:To investigate the occlusion plane changes and the mandibular plane angle changes in the adults with Angle class Ⅱ division 1 hyperdivergent malocclusion treated by different anchorages.METHODS:40 patients (aged 18-30 years) with Angle class Ⅱ division 1 hyperdivergent malocclusion had the 4 1st premolars extracted and were treated with mini-implant anchorage (Group 1,n=20) and conventional anchorage(Group 2,n=20) respectively.Pre-treatment and post-treatment cephalograms were collected and related indexes were measured,data were analyzed.RESULTS:After treatment,in group 1:U1-PP was intruded by (0.62±0.62)mm (P<0.05),SNA decreased by(0.93±1.09)°(P< 0.05).In group 2:the U1-PP was extruded by(0.94±1.27)mm (P<0.05),the U6-PP extruded by(1.41±1.00)mm (P<0.05),the U6-SV mesially moved by (2.56±1.46)mm (P<0.05),SNA decreased by (0.85±0.74)°(P<0.05),SNB decreased by (1.23±0.88)°(P<0.05),the OP-SN angle increased by (2.40±1.48)°(P<0.05),Y angle increased by (1.20±1.36)°(P<0.05),MP-SN and GoGn-SN were increased by (0.08±0.08)° and (0.03±0.94)° respectively (P<0.05).The change of other indexes was not statistically defferent.CONCLUSION:Mini-implants anchorage is more effective than traitional ancohorage in the control of the occlusion plane and the mandibular plane for the treatment of adults with Angle class Ⅱ division 1 hyperdivergent malocclusion.
