摘要
目的:探讨牙周炎患者外周血中TNF-α、IL-6、IL-10和HMGB-1水平与口臭值(OR)和菌斑指数(PLI)的相关性.方法:采集20例重度慢性牙周炎伴口臭患者(Ⅰ组)、12例单纯口臭患者(Ⅱ组)、12例轻度牙周炎患者(Ⅲ组)和8例正常人(对照组)的外周血,并通过ELISA双抗夹心法测定其血清中TNF-α、IL-6、IL-10和HMGB-1的含量;采用鼻闻法检测各患者的OR,并检测记录其PLI;数据用SPSS 17.0软件进行统计分析.结果:Ⅰ组和Ⅲ组患者的TNF-α、IL-6和HMGB-1水平均显著高于对照组(P<0.05);Ⅰ组患者的各项指标均显著高于Ⅱ组(P<0.05);Ⅰ组和Ⅱ组患者的HMGB-1水平均高于Ⅲ组(P<0.05).Ⅱ组患者的OR显著高于Ⅲ组(P<0.05);3组患者的OR均与其PLI呈明显正相关(P<0.05);Ⅰ组患者的OR和PLI均与TNF-α、IL-6、HMGB-1呈明显正相关(P<0.05);Ⅱ组患者的PLI与其HMGB-1呈明显正相关(P<0.05).Ⅲ组患者的OR和PLI均与其HMGB-1呈明显正相关(P<0.05).结论:TNF-α、IL-6和HMGB-1,特别是HMGB-1的超表达和过度释放可能是引起牙周炎慢性化和顽固性口臭的重要因素.
Abstract
AIM: To assess the levels of TNF-α,IL-6,IL-10 and HMGB-1 in peripheral blood from the patients with chronic periodontitis,and the correlation with halitosis and dental plaque index(PLI).METHODS: 20 patients with severe chronic periodontitis and oral malodor(group Ⅰ),12 with simple oral malodor(groupⅡ),12 with mild chronic periodontitis without halitosis(group Ⅲ) and 8 healthy subjects without periodontitis and halitosis (control group) were included.The levels of TNF-α,IL-6,IL-10 and HMGB-1 in peripheral blood were examined with double antibody sandwich ELISA.The organoleptic rating (OR) was evaluated by organoleptic method and PLI was measured.The data was analyzed by SPSS 17.0.RESULTS: The levels of TNF-α,IL-6 and HMGB-1in peripheral blood of groupⅠand Ⅲ were higher than those of control group (P<0.05).The levels of TNF-α,IL-6,IL-10 and HMGB-1 of groupⅠ were higher than those of groupⅡ(P<0.05).The HMGB-1s levels in groupⅠand Ⅱwere higher than that those in group Ⅲ(P<0.05).The PLI level of groupⅠwas higher than that of group Ⅱ(P<0.05).In groupⅡ the OR level was higher than that in group Ⅲ(P<0.05).In group Ⅲ,OR was positively correlated with PLI (P<0.05).In group Ⅰ,OR and PLI were positively correlated with TNF-α,IL-6 and HMGB-1(P<0.05).In group Ⅱ PLI was positively correlated with HMGB-1 (P<0.05).OR and PLI levels in group Ⅲ were positively correlated with HMGB-1 (P<0.05).CONCLUSION: Over-expression and release of TNF-α,IL-6,HMGB-1,and especially HMGB-1,may contribute to chronic periodontitis and stubborn bad breath.
