摘要

目的:探讨2型糖尿病对牙周炎患者龈沟液中细胞因子IL-1β、IFN-γ、IL-10、PGE2含量的影响.方法:选择2型糖尿病伴牙周炎患者8例(A组)、糖尿病患者8例(B组)、牙周炎患者8例(C组)、牙龈炎患者8例(D组)、全身和牙周正常者8例(E组)为研究对象,分别测定各组龈沟液中的IL-1β、IFN-γ、IL-10、PGE2含量,并同时测定其牙周临床指标.结果:A、C组的龈沟液重量、PD、AL分别大于B、E组(P<0.05);A、C组的IFN-γ、IL-1β水平分别高于B、D、E组(P<0.05),A组高于C组(P<0.05);A、C组PGE2水平分别高于B、D、E组(P<0.05),A、C组差异无统计学意义(P>0.05);A、C组IL-10水平分别低于B、D、E组(P<0.05),A组低于C组(P<0.05);IL-1β、IFN-γ、PGE2含量与龈沟液重量、PD、AL均呈正相关(r>0.429 6,P<0.05),而IL-10则与之呈负相关(r<-0.227 5,P<0.05).结论:龈沟液中IL-1β、IFN-γ、IL-10、PGE2含量的改变受全身、局部因素共同影响,糖尿病可促进牙周局部组织炎症的发展.

Abstract

AIM: To investigate the effects of type 2 diabetes on the levels of Il-1β,IFN-γ,IL-10 and PGE2 in gingival crevicular fluid(GCF) in patients with periodontitis.METHODS: 8 patients with type 2 diabetic and periodontitis(group A),8 with type 2 diabetic(group B),8 with periodontitis(Group C),8 with gingivitis(group D),and 8 healthy subjects (group E) were included.Clinical periodontal measures were recorded and the levels of IL-1β,IFN-γ,IL-10 and PGE2 in GCF were measured.RESULTS: The weight of GCF was higher in group A,C and D than that in group B and E(P<0.05).The levels of IFN-γ,IL-1β were higher in group A and C than those in group B,D and E(P<0.05),while group A presented even higher concentrations than group C(P<0.05).Group A and C demonstrated greater PGE2 level than group B,D and E(P<0.05),there was no significant difference between group A and C.The level of IL-10 was lower in groups A and C that in group B,D and E(P<0.05),group A showed even lower IL-10 level than group C(P<0.05).There was a positive correlation between the levels of IL-1β,IFN-γ,PGE2 and the weight of GCF,PD and AL(r>0.429 6,P<0.05),while IL-10 level presented a negative correlation with the weight of GCF,PD and AL (r<-0.227 5,P<0.05).CONCLUSION: The change of levels of IL-1β,IFN-γ,IL-10 and PGE2 in GCF is a consequence of systemic and local factors,and type 2 diabetes enhances periodontal inflammatory response.
