摘要

目的:采用有限元分析法分析不同厚度垫底材料对活髓牙充填后应力分布的影响.方法:选择解剖形态正常的离体上颌第一前磨牙,并于螺旋CT扫描数据,建立垫底厚度分别为0(无垫底材料的对照组)、0.5、1.0、1.5 mm的邻面洞三维有限元模型.用workbench 15.0有限元分析软件分析生理性力作用下,不同垫底厚度对牙体组织、垫底材料应力分布的影响.结果:当垫底厚度为0.5 mm时,牙体组织体和垫底层的最大/最小主应力值均最小,而当垫底厚度由1.0 mm增加到1.5 mm时,牙体组织、充填体和垫底层的应力值均明显增大.在垫底层厚度为1.5 mm时,垫底层的最小主应力超过了牙本质的抗压强度(11.4 MPa);垫底层的最大主应力值与充填体抗拉强度值相差较小.结论:从保护牙体组织的角度考虑,临床上对活髓牙进行充填时,采用0.5~1.0 mm的垫底厚度较为适宜.

Abstract

AIM:To analyze the effect of different thickness of the rebase material on the stress distribution of the teeth with vital pulp after filling.METHODS:The normal maxillary first premolars were selected and rebased by self-curing glass-ionomer with the thickness of 0.5 mm,1.0 mm and 1.5 mm respectively.The physiological role of occlusal force and the stress distribution of the tooth tissue and rebase were analyzed by workbench 15.0 finite element analysis software.RESULTS:When the bottom thickness was 0.5 mm,the maximum/minimum principal stress of the tooth tissue,the filling body and the bottom layer was the smallest,while the bottom thickness increased from 1.0 mm to 1.5 mm,the stress value of the teeth,the filling body and the bottom layer increased significantly.The minimum principal stress of the bottom layer in the 1.5 mm group was higher than that of the dentin compressive strength(11.4 MPa);the maximum principal stress value of the bottom layer showed little difference with the tensile strength of the filling body.CONCLUSION:From the point of view of the protection of the dental tissue,the appropriate rebase thickness should be 0.5 ~ 1.0 mm.
