广泛型侵袭性牙周炎基础治疗前后牙槽骨变化的CBCT评价

摘要
目的:通过锥形束CT(CBCT)分析广泛型侵袭性牙周炎(GAgP)基础治疗前及治疗后3、6个时牙槽骨的变化.方法:收集GAgP患者10例,共155个牙930个位点,分别于基础治疗前和治疗后3、6个月拍摄CBCT,并测量其探诊深度(PD)、附着丧失(CAL)、出血指数(BI).运用CBCT附带软件InvivoDental分析测量治疗前及治疗后3、6个月的牙槽嵴顶平面灰度值(HU)及牙槽嵴顶高度(%).结果:治疗后3、6个月时,所有患者的BI、PD、CAL均较治疗前显著改善(P<0.05).牙槽嵴顶HU值在治疗前及治疗后3、6个月分别为658±321、708±181、694±193,治疗后3、6个月分别与治疗前相比均有显著增高(P<0.05);牙槽嵴顶高度(牙根长度的%)在治疗前及治疗后3、6个月分别为(61.96±19.93)%、(64.68±19.60)%、(66.79±20.02)%,治疗后6个月相对治疗前显著增高(P<0.05).结论:牙周基础治疗可促进GAgP导致的牙槽骨缺损的修复.
Abstract
AIM:To analyze the alveolar bone change before and after treatmen of generalized aggressive periodontitis(GAgP) by cone beam compute fomography.METHODS:10 cases of GAgP with 155 teeth were examined by CBCT at baseline,3 and 6 months after periotontal initial therapy,probing depth (PD),clinical attachment loss(CAL),blooding index(BI) were measured at the same time.The Hounsfield units(Hu) value and the height of crest of alveolar bone were measured by Invivo Dental software.Data were analyzed with IBM SPSS Statistics 21.0.RESULTS:There was significant improvement for all parameters included BI,PD and CAL after initial therapy (P <0.05).The Hu value of the crest of alveolar bone at baseline,3 and 6 months after treatment was 658 ± 321,708 ± 181 and 694 ± 193 respectively(before treatment vs after treatment,P < 0.05).The height of alveolar bone (% root length) at base line,3 and 6 months after treatment was (61.96±19.93)%,(64.68 ± 19.60)%,and (66.79 ± 20.02) % respectively(base line vs 6 months after treatment,P < 0.05).CONCLUSION:Periodontal initial therapy can promote the restoration of alveolar bone defect induced by generalized aggressive periodontitis.
