青少年正畸治疗患者牙龈卟啉单胞菌及其毒力因子FimA基因型的变化研究

摘要
目的:分析牙龈卟啉单胞菌(Pg)及其毒力因子FimA在青少年正畸患者治疗过程中的变化.方法:随机收集青少年需正畸患者中牙龈健康者61例,分别采集正畸矫治器粘戴前及粘戴后第1、2、3、6个月时的龈沟液样本,并记录其牙龈指数,用16S rDNAPCR技术检测各样本中Pg及其毒力因子FimA(6种类型)的表达,并对实验数据进行统计学分析.结果:与矫治前相比,矫治后3个月Pg和FimA基因Ⅱ、Ⅳ、Ⅴ型均显著增加(P<0.05).Pg和6种基因型的阳性检出率在佩戴矫治器前3个月呈增长趋势,并在第3个月时达到最高水平,随后开始下降;到第6个月时,Ⅰ、Ⅰ b型恢复到了矫治前的状态.%及Ⅰ、Ⅰb、Ⅱ、Ⅳ、Ⅴ型与牙龈指数呈正相关关系(P<0.05).结论:青少年患者在正畸治疗过程中,矫治器戴入早期可导致Pg及FimA各基因型的增加,诱发牙龈炎症.
Abstract
AIM:To explore the changes of Porphyromonas gingivalis (Pg) and its virulence factor FimA in the adolescent patients underwent ortIhodontic treatment.METHODS:61 orthodontic teenagers with healthy gingiva underwent orthodontic treatment.Givgival crevicular-fluid(GCF) of the subjects was collected before and 1,2,3 and 6 month after treatment respectively,simultaneously the gingival index (GI) of one-to-one correspondence was examined,Pg and its virulence factor FimA in GCF of each sample were detected by 16SrDNA PCR technique.SPSS 20.0 software package was used to analyze the data.RESULTS:3 months after treatment.Pg and FimA Genotype Ⅱ,Ⅳ and Ⅴ in GCF were increased (P < 0.05).The positive growth of Pg and the 6 genotypes in the first 3 monthsafter treatment were increased,and the highest level appeared at the third month,then gradually decreased and returned to the prior treatment state in the sixth month.Significant positive correlation was observed between Pg and GI and FimA genotype Ⅰ,Ⅰb,Ⅱ,Ⅳ and Ⅴ respectively (P < 0.05).CONCLUSION:During the orthodontic treatment in adolescents,the increase of Pg and FimA can be caused by the wearing the appliance in the early stage,that may induce gingival inflammation.
