Analysis of health literacy level and influencing factors of residents in Hebei Province in 2020

目的 了解2020年河北省居民健康素养水平和变化趋势,为制定河北省卫生健康相关政策提供科学依据.方法 采用分层多阶段与人口规模成比例(probability proportional to size,PPS)抽样相结合的方法,抽取全省15～＜70岁的常住人口进行问卷调查,统计方法采用复杂抽样模块下的描述性分析、x2检验、logistic回归进行现况和影响因素分析.结果 河北省共调查城乡居民20535人,居民的健康素养水平总体为26.68％,城市居民为28.30％,农村居民为26.20％.多因素logistic回归分析结果表明,年龄、职业、文化程度是河北省居民健康素养水平的影响因素.居民的文化程度越高,健康素养水平越高.以不识字或识字很少的居民为参照,初中、高中/职高/中专、大专、本科及以上居民的健康素养水平[比值比(odds ratio,OR)=3.413、95％置信区间(confidence interval,CI):1.907～6.108,OR=6.039、95％CI:3.176～11.483,OR=8.263、95％CI:4.167～16.384,OR=13.136、95％CI:6.057～28.488].结论 河北省居民健康素养水平高于全国平均水平,但仍处于较低水平.应以提高农村地区、低文化程度和老年群体的健康素养水平为重点,采取有针对性的健康教育和促进干预措施.
Objective To understand the level and trend of health literacy of residents in Hebei Province in 2020,and to provide a scientific basis for the formulation of health-related policies in Hebei Province.Methods A combination of stratified multi-stage and probability proportional to size(PPS)sampling method was used to select the permanent residents aged 15 to＜70 years old in the province for the questionnaire survey.Descriptive analysis,Chi-square test and logistic regression under the complex sampling module were used for statistical methods to analyze the status quo and influencing factors.Results A total of 20 535 urban and rural residents were surveyed in Hebei Province.The overall health literacy level of residents was 26.68％,28.30％for urban residents and 26.20％for rural residents.Multivariate logistic regression analysis showed that age,occupation and education level were the influencing factors on the health literacy of residents in Hebei Province.The higher the education level of residents,the higher the level of health literacy.Taking residents who were illiterate or had little literacy as a reference,the health literacy level of residents in junior high school,senior high school/vocational high school/junior college,college,bachelor's degree and above[odds ratio(OR)=3.413,95％confidence interval(CI):1.907-6.108;OR=6.039,95％CI:3.176-11.483;OR=8.263,95％CI:4.167-16.384;OR=13.136,95％CI:6.057-28.488].Conclusion The health literacy level of residents in Hebei Province is higher than the national average level,but it is still at a low level.Targeted health education and promotion interventions should focus on improving health literacy in rural areas,low education level and elderly groups.
