Objective To study the survival status and influencing factors of cases with advanced schistosomiasis in Wuhan City, and to provide a basis for the prevention and treatment of advanced schistosomiasis. Methods A retrospective cohort study was conducted to collect data on registered cases with advanced schistosomiasis in Wuhan City from 2001 to 2022.A percentage ratio was used to characterize a categorical variable.Kaplan-Meier method was applied to calculate the survival time of cases with advanced schistosomiasis.Log-rank test was used to compare the differences in survival time of cases.Cox proportional hazard model was performed to analyze the influencing factors of the survival time of the cases. Results The median age M(P25-P75)of 920 cases of advanced schistosomiasis was 58(51-66)years old, with 29.02%(267/920)of cases aged 65 years old and above,83.26%(766/920)of cases with elementary school education and below. The clinical typing was dominated by ascites type in 70.65%(650/920).The overall fatality rate was 5.24/1 000 people per month. The 1 year,5 years, and 10 years survival rates for confirmed cases were 88.85%,59.40%,and 47.30%,respectively.The median survival time was 104.680 months(95%CI:83.250-126.749).The results of multivariate Cox regression analysis showed that males, old ages, and ascitic type cases were the risk factors affecting the survival time of cases with advanced schistosomiasis in Wuhan City. Conclusion Males, old ages, and ascitic type schistosomiasis cases have a higher mortality risk.Targeted and comprehensive measures should be taken to reduce the mortality risk of cases with advanced schistosomiasis in Wuhan City,in terms of strengthening health education and publicity and clinic treatment efforts.
