Comparison of adverse reactions of simultaneous vaccination of diphtheria tetanus and pertussis vaccine and bivalent poliomyelitis vaccine in Baoding City

目的 评价2016-2019年保定市适龄儿童无细胞百白破联合疫苗(diphtheria tetanus and acellular pertussis combined vaccine,DTaP)与二价口服脊髓灰质炎减毒活疫苗(bivalent oral poliomyelitis attenuated live vaccine,bOPV)(简称"二价脊灰疫苗")同时接种的安全性.方法 采用描述性方法计算两种疫苗单独和同时接种不良反应的报告发生率、构成比等流行病学相关指标,用SPSS 22统计软件进行指标的分析比较.结果 DTaP单独接种、bOPV单独接种、DTaP和bOPV同时接种不良反应发生率分别为169.74/10万剂、14.34/10万剂、17.84/10万剂,一般反应发生率分别为168.31/10万剂、13.49/10万剂、17.12/10万剂,异常反应发生率分别为1.42/10万剂、0.85/10万剂、0.72/10万剂.DTaP和bOPV同时接种与DTaP单独接种不良反应、一般反应、异常反应的发生率的比较,差异有统计学意义(x2=4 316.267、4 302.705、19.282,P＜0.05);DTaP和bOPV同时接种与bOPV单独接种不良反应、一般反应的报告发生率比较,差异有统计学意义(x2=91.310、89.949,P＜0.05),异常反应的发生率比较,差异无统计学意义(x2=1.862,P＞0.05).结论 DTaP和bOPV同时接种未明显增加不良反应的发生风险和严重程度,安全性良好.DTaP和bOPV同时接种可提高疫苗接种的及时性,应加强宣教以提高疫苗接种的依从性,同时做好疫苗接种前儿童健康评估.
Objective To evaluate the safety of simultaneous vaccination of diphtheria tetanus and acellular pertussis combined vaccine(DTaP)and bivalent oral poliomyelitis attenuated live vaccine(bOPV)(referred to as bivalent polio vaccine)for eligible children in Baoding City from 2016 to 2019.Methods The epidemiological correlative indexs related to the reported incidence rate and proportion of adverse reactions to the two vaccines alone and concurrently were calculated by a descriptive method.The indicators were analyzed and compared using SPSS 22 statistical software.Results The incidence rates of adverse reactions after vaccination with DTaP alone,bOPV alone,DTaP and bOPV simultaneously were 169.74/100 000 doses,14.34/100 000 doses,and 17.84/100 000 doses,respectively.The incidence rates of general reactions were 168.31/100 000 doses,13.49/100 000 doses,17.12/100 000 doses,respectively.The incidence rates of abnormal reactions were 1.42/100 000 doses,0.85/100 000 doses,0.72/100 000,respectively.There were statistically significant differences in the incidence rates of adverse reactions,general reactions,and abnormal reactions between simultaneous vaccination of DTaP and bOPV and separate vaccination of DTaP(x2=4 316.267,4 302.705,19.282,P＜0.05).There were statistically significant differences in the reported incidence rates of adverse reactions and general reactions between simultaneous vaccination of DTaP and bOPV and separate vaccination of bOPV(x2=91.310,89.949,P＜0.05),while there was no statistically significant difference in the incidence rate of abnormal reactions(x2=1.862,P＞0.05).Conclusion Simultaneous vaccination of DTaP and bOPV do not obviously increase the risk and severity of adverse reactions,and the safety profile is good.Simultaneous vaccination of DTaP and bOPV can improve the timeliness of vaccination,and education should be strengthened to increase adherence to vaccination.At the same time,meanwhile,the health assessments of children should be conducted before vaccination.
