摘要
目的：研究应用中心静脉导管行胸腔闭式引流治疗ICU中突发气胸的临床疗效，为今后疗效的提升提供可参考依据。方法将66例ICU病房突发气胸的患者按照治疗方式不同分为两组，对照组（33例）以粗硅胶管行胸腔闭式引流，观察组（33例）应用中心静脉导管行胸腔闭式引流，对比两组操作时间、出血量、并发症发生等情况。结果观察组创伤口直径更小，出血量更少，操作时间及肺复张时间更短；观察组堵管、管周疼痛、伤口感染、皮下气肿发生率均较对照组低，差异均有统计学意义（P＜0．05）。结论中心静脉导管行胸腔闭式引流治疗ICU中突发气胸有创伤小、出血量少、安全性高、操作简便等优点，值得大力推广。
Abstract
Objective:To explore the ef ect closed thoracic drainage by central venous catheter to treat sudden pneumothorax in ICU .Methods:66 patients with sudden pneumothorax in ICU,were divided randomly into the control group (33 cases)and the observation group (33 cases).Patients in the control group were given closed thoracic drainage by thick silica gel cathe-ter,while 33 cases in the observation group had closed thoracic drainage of central venous catheter.After that,some indices were compared between two groups.Results:There was a significant dif erence about wound diameter,the amount of bleeding and the time of pulmonary reexpansion between the observation group and the control group.And some indices,including the retain time of tube,pain around the tube,wound infection incident and subcutaneous emphysema in the observation group were bet er than that of control group(P<0.05 ).Conclusion:Closed thoracic drainage by central venous catheter to treat sudden pneumothorax in ICU was safe and simple,with fewer traumas and fewer amount of bleeding,which should been chosen in clinical treatment for patients in ICU.
