内外固定治疗创伤性浮膝的疗效比较

摘要
目的：比较两种不同方法治疗创伤性浮膝的疗效。方法2000年6月～2012年4月68例损伤性浮膝患者根据治疗方法不同分为内固定组35例，外固定组33例。不同的分型及有无合并伤，选择不同的治疗方法和时机，并由此决定术后功能锻炼的时间和方法，将两组的疗效、预后进行分析。结果经过8个月到5．6年的随访（平均2．6年），肢体功能参照Karlstrom和Olerud[3]制定标准评价，35例内固定者，优11例，良18例，可5例，差1例，优良率82．86％（29／35）；33例外固定手术者，优9例，良12例，可9例，差3例，优良率63．64％（21／33），两组优良率比较P＜0．05。结论采取内固定并结合CPM治疗创伤性浮膝是一种较为有效的方法。
Abstract
Objective To compare the therapeutic ef ect of traumatic floating knee between with internal fixation or with external fixation.Methods Sixty eight cases with traumatic floating knee had been divided into two group basing on the diferent methods between June 2000 and April 2012.i.e.internal fixation group(n=35)and external fixation group(n=33).The diferent treatment methods and operative opportunity were selected according to their dif erent typing and fracture position.then functional exercise after operation were decide according to this plan.The outcomes of the two groups and the function of the knee joints involved were analyzed.Results Al the 68 cases had been fol owed-up an average 2.6 years (from 8 months to 5.6 years),the rateofbothexcelentandgoodis82.86%(29/35)intheinternalgroup,and63.64%(21/33)intheexternalgroup.Thediferenceoftherateofbothexcelentandgoodhadstatisticalsignifi-cance between the two group(P<0.05).Conclusion The fracture of floating knee is a kind of severe unstable wound,and to treat the fracture with internal fixation combined with CPMis an ef-fective method.
