摘要
目的：探讨胃间质瘤（GST）的 CT表现及其诊断价值。方法回顾性分析18例经手术病理及免疫组化证实的 GST的 CT和相关临床表现资料。结果 GST的CT表现为软组织肿块，肿块可向胃腔内、外或同时向腔内外突出，肿块的密度均或不均匀，大小不等，形态不一；增强后，肿块均或不均匀强化，可见中心出血囊变坏死。本组病例共检出病灶18个，均为单发，其中位于胃体部9例，胃窦部5例，同时累计胃体胃窦1例，胃底部3例；腔内生长9例，腔内外生长5例，腔外生长4例；良性12例，CT所见瘤体最大径均≤5．0cm，11例密度均匀，明显均匀强化，形态多规则，1例密度欠均，呈不均匀强化；恶性或潜在恶性6例，CT表现为肿块较大，5例最大径＞5．0cm，1例≤5．0cm，形态多不规则，内可见出血囊变坏死，部分见斑点状钙化，增强不均匀强化。结论胃间质瘤的CT表现具有一定的特点，CT对GST定位定性及鉴别诊断具有重要价值。
Abstract
Objective To analyze CT features of gastric stromal tumors(GST)and the value of CT diagnosis.Methods Clinical data and CT findings in 18 cases with GST proved pathologi-cal y were retro -spectively analyzed.Results CT findings of GST showed that the lesions were intra-luminal,extra-luminal or both in-tra-and extra-luminal soft tissue mass and showed homogeneous density or heterogeneous density on plain CT scan.Lesions showed the mass uniform or irregular strengthened,visible central hemorrhage,cystic and necrosis on enhanced CT scan.In patients with GST,18 lesions were detected and al of them were solitary.9 cases of the tumors were located in body of stomach,5 cases in gastric antrum,1 case between gastric antrum and the body of stomach and 3 cases in fundus stomach.9 cases of the soft tissue masses were intra-luminal,4 cases extra-luminal and 5 cases both in-tra-and extra-luminal .Among them 12 cases were benign and 8 cases were malignant.The benign lesions were below 5 cm in size and 11 case with wel -defined margin and displayed homogeneous enhancement,1 case den-sity in masses were uneven and displayed unhomogeneous enhancement.The malignant or pre-malignant lesions were il -defined and 5 case over 5 cm in size (1 case below 5 cm)with hem-orrhage,cystic ,necrosis and stippled calcification within the tumors,.Conclusion CT findings of GST has some features,which can detect and precisely localize GST,and it is helpful in dif eren-tiation of benign or malignant GST.
