摘要
目的：观察活血化瘀中药丹参联合甲氨蝶呤治疗对银屑病关节炎患者临床症状、血沉及 C 反应蛋白的影响。方法将31例银屑病关节炎患者按随机数字表随机分为对照组（16例）及治疗组（15例）。对照组给予患者甲氨蝶呤15mg 口服，每周一次，治疗组在对照组基础上增加静脉注射丹参注射液。每组均治疗6周，观察治疗前后相应症状、血沉（ESR），C 反应蛋白（CRP）及白介素－17（IL －17）的变化。结果1．关节疼痛症状：两组治疗后均比治疗前好转（P ＜0．05）；治疗组比对照组效果更明显（P ＜0．05）。2．关节肿胀症状：两组治疗后均比治疗前好转（P ＜0．05）；治疗组也比对照组效果更明显（P ＜0．05）。3．屈伸不利症状：两组治疗后均比治疗前好转（P ＜0．05）；治疗组比对照组效果未出现明显差异（P ＞0．05）。4．CRP，ESR 以及 IL －17：两组治疗后均比治疗前好转（P ＜0．05）；治疗组的 IL －17用药后相比对照组用药后出现明显差异（P ＜0．05），治疗后的两组 CRP 及ESR 未出现差异（P ＞0．05）。结论活血化瘀中药丹参联合甲氨蝶呤治疗对银屑病关节炎患者的关节疼痛及肿胀有着更好的改善作用，对血清中 IL －17有更明显的作用，其相关机制还有待进一步研究。
Abstract
Objective:To observe the clinical ef ects of Salvia miltiorrhiza (Danshen)plus Methotrexate for psoriatic arthritis.Methods:Thirty -one psoriatic arthritis patients were randomized into two groups:the Danshen group(15 cases)which was treated by Salvia injection plus Methotrexate,the control group(16 cases)which was treated only by Methotrexate.The con-trol group were treated with Methotrexate (15mg/w),the Danshen group were treated with Salvia injection (40ml /d)plus Methotrexate.Each group was treated for 6 weeks.Main symptoms, CRP,ESR and IL -17 scores were evaluated. Results:1.For pressure pain of arthritis,joints swel ing and joint motion in the same group ,there were dif erences between pre -treatment and post treatment(P <0.05).For pressure pain of arthritis and joints swel ing post treatment,there were dif erences between the two groups (P <0.05).For joint motion there was no significant dif erence between two groups post treatment (P>0.05).2.Compared with pre -treatment,ESR,CRP and IL -17 score showed significant improvement in both two groups (P <0.05).IL -17 scores showed significant dif erence between two groups post treatment(P <0.05).Conclusion:Salvia miltiorrhiza (Danshen)plus Methotrexate can improve some clinical symptoms (pressure pain of arthritis,joints swel ing and joint mo-tion),ESR and CRP score in psoriatic arthritis.
