摘要
目的：观察去瓣 Lasek 与去瓣 Epi －Lasik 的临床效果、安全性、优越性，以此指导临床选择手术方案。方法采用前瞻性研究，选取100例（200眼）近视患者，患者单盲前提下，术中随机采用一眼行去瓣 Lasek，另一眼行去瓣 Epi －Lasik，术前球镜度（sphere equivalent，SE）为－2．25～－5．50D，平均（－2．58±1．66）D，柱镜－0．50～－1．75D，平均（－1．00±0．25）D。术后非参与手术医生（确保患者与医生对手术方式双盲）对上皮愈合情况，术后7天对疼痛主观评定，术后2周、1月、3月行裸眼视力、裂隙灯下 Haze 检查，并进行统计学比较和分析。结果去瓣 Lasek 与去瓣 Epi －Lasik 在临床效果、手术安全性、优越性方面均无统计学差异，去瓣 Epi －Lasik 因机械刀的使用，存在一定的制瓣风险，手术时间相对较长。结论去瓣 Lasek 与去瓣 Epi －Lasik 临床效果相似，去瓣 Lasek 在安全性及手术效率方面似乎更具优越性，去瓣 Lasek 取代去瓣 Epi －Lasik 值得进一步探讨。
Abstract
Purpose To observe the clinical ef ect,safety and advantages of of -flap LASEK and of -flap EPI -LASIK,is order to guide the clinical choice of operation scheme Methods By means of prospective study,we choose 100 patients(200 eyes)Under the premise of patients’single blind,to do the operation,during which one eye with of -flap LASEK and the other with of -flap EPI -LASIK randomly.Before operation,the sphere equivalent(SE)is -2.25 ~-5.50D,-2.58 ±1.66 D in average,and the Cylindrical lens is -0.50 ~-1. 75D,-1.00 ±0.25D in average.After the operation,doctors who haven’t participated in the operation (To ensure that patients and doctors double blind on the operation mode )observe the Epithelial healing,make subjective assessment of the patient’ache at the point of 7 days after the operation,and do the Visual acuity examination by Haze at the point of 2 weeks,1 month and 3 months after the operation to make statistical comparison and analysis.Results There is no statistical dif erences in clinical ef ect,operation safety and superiority between of -flap LASEK and of -flap EPI -LASIK.Because of using mechanical scalpel,it is somewhat risky during the operation,and it takes more time.Conclusion The clinical ef ect is similar between of -flap LASEK and of -flap EPI -LASIK,on the hand of safety and operation ef iciency,of -flap LASEK has more advantages.It’s worth to make further study on of -flap LASEK displacing of -flap EPI-LASIK.
