摘要
目的：研究两种骨粉在拔牙窝保存术中的临床效果，比较不同骨代材料恢复和重建拔牙窝处软硬组织的能力。方法：将2013年5月－2014年12月份在我院口腔颌面外科拔除前牙或前磨牙的60例患者，随机分为3组，进口骨粉组和国产骨粉组于拔牙后分别应用 Bio －Oss、羟基磷灰石对拔牙窝进行填充，空白对照组拔牙后不进行任何干预。1、3、6个月复诊，观察拔牙窝愈合情况。拔牙前及拔牙6个月后进行口内取模灌注石膏模型并拍摄 X 线牙片，分别测量牙槽嵴的高度和宽度，进行统计学分析，P ＜0．05为差异具有统计学意义。结果：60例患者中，进口骨粉组和国产骨粉组拔牙窝均愈合良好，牙槽骨丰满，牙龈色、形、质与邻牙协调。牙槽窝骨质均得到有效保护，牙槽嵴的宽度均满足临床种植需求。而对照组牙槽嵴吸收、萎缩，高度降低、宽度显著缩小。经比较发现，进口骨粉（Bio －Oss）组各项指标变化最小，国产骨粉（羟基磷灰石）组次之，空白对照组变化最大。结论：将 Bio －Oss 或羟基磷灰石人工骨粉在拔牙后即刻植入牙槽窝中，均可以有效保护牙槽骨量，维持牙槽骨形态，顺利完成种植手术，软硬组织美学效果良好。国产骨粉更加经济和实用，适经济欠发达地区推广。
Abstract
Objective:To study the two kinds of bone in the extraction socket preservation inclinical ef ect,comparison of dif erent reconstruction ability of tooth extraction socket of soft tissue and bone restoration materials.Methods:from 2013 May -2014 year in December 60 cases of oral and maxil ofacial surgery in our hospital were anterior teeth or premolars of patients, were randomly divided into 3 groups,imported meal group and domestic meal groupafter extraction respectively using Bio -Oss,hydroxyapatite on the sockets were fil ed,the blank control group after tooth extraction without any intervention.1,3,6 months,to observe the healing of extraction sockets.Before extraction and extraction after 6 months of intraoralmodulo plaster casting and X -ray radiograph,the alveolar ridge height and width were measured,by statistical analysis,P <0.05,the dif erence was statistical y significant.Results:in 60 cases,bone and bone meal group group of imported domestic sockets healed wel ,alveolar bone plump,gingival,shape,quality and coordination of adjacent teeth.The alveolarbone have been ef ectively protected,the alveolar ridge width can meet the clinical needs of planting.While the control group of alveolar bone resorption,atrophy,reduced height,widthwas significantly reduced.By comparison,the im-port of bone (Bio -Oss)group,the indexes ofthe smal est,domestic bone (hydroxyapatite)group,blank control group,the biggest change.Conclusion:Bio -Oss or hydroxyapatite artificial bone in implanted into the socket immediately after extraction,can ef ectively protect the alveolar bone,maintain alveolar bone morphology,the successful completion of planting operation,good aesthetic ef ect of soft tissue.Domesticmeal is more economical and practical,suitable for promotion in underdeveloped areas.
