摘要
目的：探讨分期护理在肝硬化合并上消化道出血患者中的应用价值。方法：本研究于2014年1月～2014年12月对收治的肝硬化合并上消化道出血患者给予补充血容量、护肝、止血、支持治疗的基础上进行运用奥曲肽联合奥美拉唑治疗和分期护理。结果：本文患者出血量大，速度迅猛，出血量785～7150ml，平均出血量（3125．54±1025．36）ml；72例患者中出现呕血、便血患者66例，单纯便血6例。全组病人病情危重，抢救治疗难度极大，但是大多数均获得成功救治。共抢救成功48例，占94．44％（68／72），无窒息、误吸发生；病死4例，占5．56％（4／72），死亡原因多为严重并发症，2例患者死于合并多脏器功能衰竭，2例患者因抢救不及时在入院治疗时死去。结论：密切观察患者病情变化，按照分期进行了护理干预，严格遵守输血规程，及时给予抢救和护理，分期护理干预可提高肝硬化合并上消化道出血患者的治愈率。
Abstract
Objective:To investigate the application value of staging nursing in cirrhosis with upper digestive tract hemorrhage patients.Methods:From January 2014 year to December 2014 year the liver cirrhosis complicated with upper gastrointestinal hemorrhage patients were admit ed with supplement the blood volume,protecting liver,hemostasis and support treatment on the basis of the use of octreotide combined with omeprazole in treatment and nursing.Results:the patients with haemorrhage amount was large,the speed was rapid,and the amount of bleeding was 785 ~7150ml,average bleeding volume (3125.54 ±1025.36)ml.66 cases of patients with hematemesis,hematochezia in 72 patients,only 6 cases of hematochezia.Al the patients in critical condition,treatment is dif icult,but most were successful treatment.A total of 48 cases of successful treatment,accounted for 94.44% (68 /72),without asphyxia,aspiration;4 cases died,accounting for 5.56% (4 /72),died of serious complications,2 patients died of multiple organ failure,2 patients died in hospital due to the rescue is not timely treatment.Conclusion:Closely observe the patients’condition changes,according to the staging of the nursing intervention,strictly abide by the rules of blood transfusion,timely treatment and care,nursing interven-tion can improve the cure rate of patients with cirrhosis with upper digestive tract hemorrhage.
