Correlation between Neutrophil-to-high-density Lipoprotein Cholesterol Ratio and Coronary Artery Stenosis in Patients with Type 2 Dia-betes Mellitus
Objective To explore the correlation between neutrophil-to-high-density lipoprotein cholesterol ratio(NHR)and coronary artery stenosis in patients with type 2diabetes mellitus.Methods A retrospective analysis was conducted on 452 patients with type 2diabetes mellitus(T2DM),who underwent coronary angiography at Xinhua Hospital affiliated to Shanghai Jiao Tong University School of Medicine from October 2019 to January 2024.Clinical data,laboratory findings,and coronary angiography results were collect-ed.The patients were divided into study group(Gensini score>0,n=364)and control group(Gensini score=0,n=88)according to Gensini score.Subsequently,the study group was further categorized into subgroups based on the severity of coronary artery stenosis:mild stenosis(Gensini score<20 points,n=134),moderate stenosis(20 points≤Gensini score ≤40 points,n=122),and severe ste-nosis(Gensini score>40 points,n=108).The data among all groups were compared,and the risk factors of coronary artery stenosis in T2DM patients were investigated by multivariate Logistic regression analysis and Spearman correlation analysis,and the predictive value of NHR in severe coronary artery stenosis in T2DM patients was evaluated by receiver operating characteristic(ROC)curve.Results Fast-ing blood glucose,HbA1c,and NHR in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).Multivariate Logistic regression analysis indicated that NHR(OR=2.691,95%CI:1.509-4.796,P<0.001)could serve as independent risk factors for coronary artery stenosis in patients with T2DM.There was significant difference in NHR among mild stenosis group,moderate stenosis,and severe stenosis(P<0.05).Spearman correlation analysis showed a positive correla-tion between NHR and the severity of coronary artery stenosis in patients with T2DM(r=0.336,P<0.001).The area under the ROC curve for NHR predicting severe coronary artery stenosis in patients with T2DM was 0.687(95%CI:0.604-0.769,P<0.001).When the cut-off value of NHR was 5.79,its diagnostic efficiency was the highest,with a specificity of 83.7%,and the sensitivity was 48.1%.Conclusion The NHR can serve as an independent risk factor for coronary artery stenosis in patients with T2DM.There is a positive correlation between NHR and the severity of coronary artery stenosis in patients with T2DM.NHR has predictive value for severe coronary artery stenosis in patients with T2DM.
