摘要
目的:研究急性脑梗死患者诊断中,颈部血管超声诊断(CUS)的应用价值,对其颈动脉病变情况进行评估.方法:收集2021 年 10 月—2023年10月滕州市工人医院收治的100例急性脑梗死患者的临床资料,所有入组者均进行CUS诊断,诊断金标准为数字减影血管造影(DSA),分析CUS对于急性脑梗死的诊断价值.结果:CUS病变检出率与金标准差异无统计学意义(P>0.05);CUS诊断颈动脉病变狭窄程度分析结果显示,100 例患者中,颈动脉病变 51 例(51.00%),其中管腔轻度、中度、重度狭窄以及完全闭塞分别为 18 例、14 例、12 例、7 例,占比分别为 18.00%、14.00%、12.00%、7.00%;CUS诊断的阴性预测值、阳性预测值、特异度、灵敏度、准确率均较高,分别为 97.92%(47/48)、96.15%(50/52)、95.92%(47/49)、98.04%(50/51)、97.00%(97/100),Kappa值=0.940,与金标准一致性极好.结论:CUS的诊断价值较高,与DSA诊断的检出率差异较小,可以有效分析颈动脉病变狭窄程度,具有较高的准确率、阴性预测值、灵敏度、特异度、阳性预测值,对后续针对性治疗具有积极意义,推广及应用价值高.
Abstract
Objective to study the application value of carotid Doppler ultrasound(CUS)in the diagnosis of patients with acute cerebral infarction,and to evaluate their carotid artery lesions.Methods Clinical data of 100 patients with acute cerebral infarction admitted to Tengzhou Workers'Hospital from October 2021 to October 2023 were collected.All enrolled patients were diagnosed with CUS.The gold standard for diagnosis was digital substractionangiography(DSA),to analyze the diagnostic value of CUS in acute cerebral infarction.Results There was no statistically significant difference between the detection rate of lesions by CUS and the gold standard(P>0.05);the results of the analysis of the degree of stenosis of carotid artery lesions diagnosed by CUS showed that there were 51 cases of carotid artery lesions in the 100 patients(51.00%),in which the lumens of the carotid arteries were mildly,moderately,severely stenosed,and totally occluded in 18,14,12,and 7 cases,respectively,with a percentage of 18.00%,14.00%,12.00%,and 7.00%respectively;the negative predictive value,positive predictive value,specificity,sensitivity,and accuracy of CUS diagnosis were high 97.92%(47/48),96.15%(50/52),95.92%(47/49),98.04%(50/51),and 97.00%(97/100),Kappa value=0.940,highly consistent with the gold standard.Conclusion The diagnostic value of CUS is high,and the detection rate difference with DSA diagnosis is small,which can effectively analyze the degree of stenosis of carotid artery lesions,with high accuracy,negative predictive value,sensitivity,specificity,positive predictive value,which has positive significance for the subsequent targeted treatment,and has high promotion and application value.
