影像组学在经导管动脉化疗栓塞术治疗原发性HCC中的研究进展

原发性肝细胞癌(HCC)是常见的肝脏富血供恶性肿瘤,由于早期缺乏特异性的症状及体征,绝大多数患者就诊时已经错过手术根治的机会,对于不可切除的原发性HCC,经导管动脉化疗栓塞术是首选的治疗手段.影像组学是一种可以高通量、深层次地挖掘并利用医学图像中的信息,从而反映肿瘤的异质性等细胞生物学特征的技术,在原发性HCC的诊断、鉴别诊断、治疗反应及生存预后等方面展现出独特的优势.本文主要就影像组学在经导管动脉化疗栓塞术对原发性HCC的疗效预测、复发预测及生存预测等方面的应用进展进行阐述,并对其存在的问题及未来研究的方向进行探讨和展望.
Primary hepatocellular carcinoma(HCC)is the most common malignancy with rich blood supply in the liver.Due to the lack of specific symptoms and signs in the early stage,the vast majority of patients have lost the opportunity for radical surgical treatment at the time of visit.For unresectable primary HCC,transcatheter arterial chemoembolization is the preferred treatment.Radiomics is a technology that can mine and use information from medical images in high throughput and depth to reflect tumor heterogeneity and other cell biological characteristics,and has shown unique advantages in the diagnosis,differential diagnosis,treatment response and survival prognosis of primary HCC.This article mainly reviewed the application progress of radiomics in the prediction of efficacy,recurrence and survival of transcatheter arterial chemoembolization for primary HCC,and discussed the existing problems and future research directions.
