摘要
目的:探讨多层螺旋CT检查在肝硬化诊断中的应用价值及相关CT参数分析.方法:选取枣庄市山亭区人民医院 2022 年1 月—2023 年 1 月收治的疑似肝硬化患者 80 例,所有患者均行多层螺旋CT检查,以肝穿刺活检病理检查为金标准,分析多层螺旋CT检查在肝硬化诊断中的应用价值.结果:80 例疑似肝硬化患者,肝穿刺活检病理检查结果,阳性 67 例,阴性 13 例;肝功能分级A级58 例,B级 6 例,C级 3 例.经多层螺旋CT检查显示,阳性 66 例,阴性 14 例,多层螺旋CT检查肝硬化的准确率 93.75%,灵敏度95.52%,特异度 84.62%,阳性预测值 96.97%,阴性预测值 78.57%,Kappa值=0.777,与病理检查结果一致性良好;多层螺旋CT检查肝功能分级结果显示,A级 56 例,B级 7 例,C级 4 例,检出准确率 91.04%(61/67).肝功能C级体表面积大于A、B两组,B组肝脏体积大于A、C组,差异有统计学意义(P＜0.05).结论:多层螺旋CT检查在肝硬化诊断中准确率较高,能够准确获取患者肝脏体表面积以及肝脏体积等数据,有效显示肝功能分级,为后期治疗提供依据,临床应用价值显著.
Abstract
Objective To explore the application value of multi-slice spiral CT in the diagnosis of cirrhosis and the analysis of relevant CT parameters.Methods A total of 80 patients with suspected cirrhosis admitted to Zaozhuang City Shanting District People's Hospital from January 2022 to January 2023 were selected.All patients underwent multi-slice spiral CT examination.The application value of multi-slice spiral CT examination in the diagnosis of cirrhosis was analyzed using liver puncture biopsy as the gold standard.Results In 80 patients with suspected cirrhosis,the results of liver biopsy were positive in 67 cases and negative in 13 cases.The liver function grade was A in 58 cases,B in 6 cases,and C in 3 cases.Multi-slice spiral CT showed that 66 cases were positive and 14 cases were negative.The accuracy of multi-slice spiral CT for liver cirrhosis was 93.75%,sensitivity was 95.52%,specificity was 84.62%,positive predictive value was 96.97%,negative predictive value was 78.57%,Kappa value=0.777,and the consistency was excellent with pathological examination results.The results of liver function classification by multi-slice spiral CT showed that 56 cases were grade A,7 cases were grade B,and 4 cases were grade C,with the detection accuracy of 91.04%(61/67).The body surface area of grade C liver function was higher than that of group A and B,and the liver volume of group B was higher than that of group A and C,the difference was statistically significant(P<0.05).Conclusion Multi-slice spiral CT has a high accuracy in the diagnosis of cirrhosis,which can accurately obtain the data of liver body surface area and liver volume of patients,effectively display the liver function grade,provide basis for later treatment,and have significant clinical application value.
