摘要

目的:探究腰椎间盘突出症磁共振成像(MRI)及计算机断层扫描(CT)影像学特征及诊断价值.方法:选取 2020 年 8 月—2022年9月于乳山市中医院就诊的88例疑似腰椎间盘突出症的患者,所有患者均行CT及MRI扫描,以手术结果作为诊断"金标准",分析CT及MRI在腰椎间盘突出症中检出情况、影像学特征、诊断效能.结果:CT及MRI检查在腰椎间盘突出症中椎间盘脱出、椎间盘突出、椎间盘游离、椎间盘膨出检出率准确率对比,差异无统计学意义(P>0.05);MRI扫描在椎间盘变性、神经根受压、硬膜囊受压、脊髓变性中的检出率均较高于CT扫描,差异有统计学意义(P<0.05).MRI检查在诊断腰椎间盘突出症中的灵敏度及准确度均高于CT检查,差异有统计学意义(P<0.05),两种检查方法特异度比较,差异无统计学意义(P>0.05).结论:CT及MRI均可鉴别诊断腰椎间盘突出症,但MRI检查准确率更高,在腰椎间盘突出症的影像学特征检出中具有较高的准确率,可作为临床鉴别诊断腰椎间盘突出症的首选影像学扫描方法,并为临床诊疗提供有效参考,值得推广应用.

Abstract

Objective To explore the imaging features and diagnostic value of magnetic resonance imaging(MRI)and computed tomography(CT)in lumbar disc herniation.Methods A total of 88 patients with suspected lumbar disc herniation who were treated in Rushan Hospital of Traditional Chinese Medicine from August 2020 to September 2022 were selected.CT and MRI scans were performed on all patients,and the results of surgery were taken as the"gold standard"for diagnosis.The detection status,imaging characteristics and diagnostic efficiency of CT and MRI in lumbar disc herniation were analyzed.Results There was no statistical difference between CT and MRI in the detection rate of disc herniation,disc herniation,disc free and disc bulging(P>0.05).The detection rate of MRI scan in disc degeneration,nerve root compression,dural sac compression and spinal cord deformation was higher than those of CT scan,with statistical difference(P<0.05).The sensitivity and accuracy of MRI in the diagnosis of lumbar disc herniation were higher than that of CT,with statistical difference(P<0.05),while there was no statistical difference in the specificity of the two methods(P>0.05).Conclusion Both CT and MRI can differentiate and diagnose lumbar disc herniation,but MRI has a higher accuracy in detecting the imaging features of lumbar disc herniation,which can be used as the first choice imaging scanning method for clinical differential diagnosis of lumbar disc herniation,and provide effective reference for clinical diagnosis and treatment,worthy of promotion and application.
