摘要

目的:分析彩色多普勒超声检查诊断乳腺良恶性肿瘤的临床价值.方法:选取 2020 年 8 月—2022 年 12 月淄博市市级机关医院收治的 100 例乳腺肿块的患者为研究对象,均采用彩色多普勒超声检查,以病理切片检查结果为金标准,评价彩色多普勒超声诊断的临床价值.结果:病理切片检查可见乳腺良性肿瘤 53 例、乳腺恶性肿瘤 47 例,彩色多普勒超声检查恶性 45 例,良性 55 例,彩色多普勒超声诊断乳腺癌的灵敏度为93.62%、特异度为98.11%、阳性预测值为97.78%、阴性预测值为94.55%,Kappa值=0.920,与金标准具有高度一致性;乳腺恶性肿瘤的超声形态学特征中形态不规则、边缘模糊、后方回声减弱、回声不均以及点状钙化灶检出率高于乳腺良性肿瘤(P<0.05);乳腺恶性肿瘤的峰值收缩速度(PSV)、阻力指数(RI)水平高于乳腺良性肿瘤(P<0.05).结论:彩色多普勒超声对乳腺癌良、恶性病变的鉴别诊断具有较高的临床价值,可早期检测出恶性病变,对选择合适的治疗方案和手术时间具有一定的指导意义.

Abstract

Objective To analyze the clinical value of color Doppler ultrasonography in the diagnosis of benign and malignant breast tumors.Methods A total of 100 patients with breast masses admitted to municipal government hospitals in Zibo City from August 2020 to December 2022 were selected as the study objects.Color Doppler ultrasound was used for all of them.The clinical value of color Doppler ultrasound diagnosis was evaluated with the results of pathological section as the gold standard.Results Pathological examination showed 53 cases of benign breast tumors and 47 cases of malignant breast tumors,45 cases of malignant breast tumors and 55 cases of benign breast tumors by color Doppler ultrasonography.The sensitivity,specificity,positive predictive value,and negative predictive value of color Doppler ultrasonography in the diagnosis of breast cancer were 93.62%,98.11%,97.78%,94.55%.Kappa value=0.920,highly consistent with the gold standard;The ultrasound morphological features detection rate of malignant breast tumors were significantly higher than those of benign breast tumors(P<0.05),including irregular shape,blurred edge,weakened posterior echo,uneven echo and dot calcification.The levels of PSV and RI in malignant breast tumors were significantly higher than those in benign breast tumors(P<0.05).Conclusion Color Doppler ultrasound has high clinical value for the differential diagnosis of benign and malignant breast cancer,and can detect malignant lesions at an early stage,which is of great significance in choosing the appropriate treatment plan and operation time.
