超声引导下腘静脉穿刺置管介入溶栓治疗对下肢深静脉血栓形成的疗效分析

摘要
目的:分析超声引导下腘静脉穿刺置管介入溶栓治疗对下肢深静脉血栓形成(DVT)的疗效.方法:选取2021年1月—2022年6月重庆医科大学附属第一医院收治的60例下肢DVT患者为研究对象.依据患者的治疗方式不同分为研究组(30例,超声引导下腘静脉穿刺置管介入溶栓治疗)和对照组(30例,常规口服药物治疗),研究组患者采用超声引导下腘静脉穿刺置管介入溶栓治疗,对照组患者单纯采用口服抗凝药物治疗.比较两组患者治疗前后,半年内下肢静脉血栓的再通率、再通疗效及远期血栓后综合征发生率.结果:研究组半年内下肢静脉再通率高于对照组(P＜0.05);研究组下肢静脉血栓半年内再通疗效评分高于对照组(P＜0.05);研究组下肢静脉血栓形成后综合征的发生率低于对照组(P＜0.05).结论:超声引导下腘静脉穿刺置管介入溶栓治疗下肢深静脉血栓能够获得更好的静脉再通率及总体疗效,并改善远期血栓后综合征发生率,具有非常重要的应用价值.
Abstract
Objective To evaluate the clinical value of ultrasound-guided popliteal vein catheter-directed thrombolysis in the treatment of lower extremity deep venous thrombosis(DVT).Methods A total of 60 patients with lower extremity DVT were selected as the research objects during January 2021 to June 2022 in the First Affiliated Hospital of Chongqing Medical University,and according to the different treatment methods of the patients,they were divided into a study group(30 cases)and a control group(30 cases),patients in the study group were treated with ultrasound-guided popliteal vein catheterization interventional thrombolysis,and patients in the control group were treated with oral anticoagulant drugs only,to compare the recanalization rate,recanalization efficacy,and long-term incidence of post thrombotic syndrome in lower limb venous thrombosis within six months between two groups of patients before and after treatment.Results The lower extremity venous recanalization rate in the study group was significantly higher than that in the control group(P<0.05);The recanalization efficacy score of lower extremity venous thrombosis in the study group was significantly higher than that in the control group(P<0.05).The incidence of post-thrombotic syndrome in the study group was significantly lower than that in the control group(P<0.05).Conclusion Ultrasound-guided popliteal vein catheterization interventional thrombolysis in the treatment of lower extremity deep venous thrombosis can achieve better venous recanalization rate and overall efficacy,and reduce the incidence of long-term post-thrombotic syndrome,which has very important application value.
