心脏超声诊断慢性心衰的参数分析

摘要
目的:分析心脏超声诊断慢性心衰的各项参数结果.方法:选取 2022 年 9 月—2023 年 9 月北京市西城区平安医院收治的 60 例慢性心衰患者作为研究对象,设为慢性心衰组;另选取 60 例同期体检人员为对照组.比较两组受检者的左房内径(LAD)、左室舒张末期内径(LVEDD)、左室射血分数(LVEF)、N末端B型利钠肽原(NT-proBNP)和不同心功能分级的相关性.结果:慢性心衰组LAD、LVEDD大于对照组,NT-proBNP水平高于对照组,LVEF低于对照组,差异有统计学意义(P＜0.05);慢性心衰组按照NYHA心功能分级为Ⅰ级18例、Ⅱ级21例、Ⅲ级14例、Ⅳ级7例,LAD、LVEDD逐级增大,NT-proBNP水平逐级升高,LVEF逐级降低,差异有统计学意义(P＜0.05).结论:心脏超声的各项参数能够准确地反映慢性心衰患者的心功能状态,为临床的诊断和治疗提供了有效的参考依据.
Abstract
Objective To analyze the results of various parameters in the diagnosis of chronic heart failure by cardiac ultrasonography.Methods 60 patients with chronic heart failure admitted to Beijing Xicheng District Ping'an Hospital from September 2022 to September 2023 were selected as the study subjects and assigned to the chronic heart failure group;Another 60 cases who underwent physical examinations during the same period were selected as the control group.The correlation between left atrial diameter(LAD),left ventricular end-diastolic diameter(LVEDD),left ventricular ejection fraction(LVEF),N-terminal pro-B-type natriuretic peptide(NT-proBNP)and different cardiac function grades was compared between the two groups.Results The levels of LAD and LVEDD in the chronic heart failure group were higher than those in the control group,NT-proBNP levels were higher than those in the control group,and LVEF was lower than those in the control group,with statistically significant differences(P<0.05).The chronic heart failure group was classified according to NYHA cardiac function grading as GradeⅠ in 18 cases,GradeⅡin 21 cases,GradeⅢ in 14 cases,and GradeⅣ in 7 cases.LAD and LVEDD increased step by step,NT-proBNP levels increased step by step,and LVEF decreased step by step,with statistically significant differences(P<0.05).Conclusion The parameters of cardiac ultrasound can accurately reflect the cardiac function of patients with chronic heart failure,and provide an effective reference for clinical diagnosis and treatment.
