摘要

目的:分析磁共振弥散加权成像(MRI-DWI)技术诊断急性脑梗死(ACI)的价值.方法:选取 2021 年 5 月—2023 年 5 月曹县人民医院收治的 93 例疑似ACI患者,均实施MRI-DWI检查,以数字减影血管造影(DSA)为金标准,探究MRI-DWI的诊断效能.结果:93 例疑似ACI患者中,DSA共检出 68 例,MRI-DWI共检出 67 例;以DSA为金标准,MRI-DWI诊断ACI的灵敏度为95.59%、特异度为92.00%、准确率为94.62%、阳性预测值为97.01%、阴性预测值为88.46%;MRI-DWI检查与金标准的一致性良好(Kappa值=0.865,P<0.001);患侧组织的表观弥散系数(ADC)值、脑血容量(CBV)低于对侧正常组织,平均通过时间(MTT)长于对侧正常脑组织(P<0.05).结论:MRI-DWI能够有效诊断出ACI,具有一定的诊断价值,临床可推广应用.

Abstract

Objective To analyze the value of diffusion-weighted magnetic resonance imaging(MRI-DWI)in diagnosing acute cerebral infarction(ACI).Methods A total of 93 patients with suspected ACI admitted to Caoxian People's Hospital from May 2021 to May 2023 were selected and all underwent MRI-DWI examination.Digital subtraction angiography(DSA)was used as the gold standard to explore the diagnostic efficacy of MRI-DWI.Results Among 93 suspected ACI patients,68 cases were detected by DSA and 67 cases by MRI-DWI.Using DSA as the gold standard,the sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of MRI-DWI in the diagnosis of ACI were 95.59%,92.00%,94.62%,97.01%and 88.46%respectively.The consistency between MRI-DWI and gold standard was excellent(Kappa value =0.865,P<0.001).The apparent diffusion coefficient(ADC)and cerebral blood volume(CBV)of the affected tissue were lower than those of the contralateral normal tissue,and the mean transit time(MTT)was longer than that of the contralateral normal tissue(P<0.05).Conclusion MRI-DWI can effectively diagnose ACI and has certain diagnostic value,which can be widely applied in clinical practice.
