摘要
目的:探究动态增强磁共振成像(DCE-MRI)预测宫颈癌同步放化疗治疗效果的价值.方法:选取2020年8月—2022年9月淄博市中心医院收治的100例宫颈癌患者为研究对象,所有患者均进行同步放化疗治疗,并行DCE-MRI扫描,分析治疗情况,并对比治疗前后DCE-MRI参数、不同疗效患者DCE-MRI参数,另分析DCE-MRI参数在宫颈癌同步放化疗治疗效果中的预测价值.结果:100例宫颈癌患者经同步放化疗治疗后,完全缓解10例,部分缓解34例,总缓解率为44.00%.治疗后,患者DCE-MRI平均表观弥散系数(ADC)、转运常数(Ktrans)、细胞外血管外空间的体积分数(Ve)均高于治疗前(P＜0.05).治疗有效(完全缓解、部分缓解)者DCE-MRI的ADC值、Ktrans、Ve均高于治疗无效(病情稳定、病情进展)者(P＜0.05).DCE-MRI的Ve、Ktrans、ADC值预测宫颈癌同步放化疗治疗效果的曲线下面积分别为0.947、0.756、0.677,灵敏度分别为0.864、0.727、0.614,特异度分别为0.893、0.732、0.625.结论:DCE-MRI在宫颈癌同步放化疗治疗效果中的预测价值较高,可为患者诊疗方案的制定提供个性化参考.
Abstract
Objective To explore the application value of dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)in evaluating the treatment effect of cervical cancer patients undergoing radiochemotherapy.Methods A retrospective analysis was conducted on the clinical data of 100 cervical cancer patients admitted to Zibo Central Hospital from August 2020 to September 2022.All patients were treated with radiochemotherapy,and DCE-MRI scans were performed,to analyze the treatment status of all patients;Compare DCE-MRI parameters and DCE-MRI parameters of patients with different therapeutic effects,and analyze the predictive value of DCE-MRI parameter changes in the treatment effect of cervical cancer.Results The total response rate of all patients treated with synchronous radiochemotherapy reached 44.00%.The post-treatment transport constant(Ktrans),average apparent diffusion coefficient(ADC),and volume fraction of extracellular space(Ve)were all higher than before the treatment(P<0.05);The ADC value,Ktrans,and Ve levels of patients with effective treatment were higher than those with ineffective treatment(P<0.05).The ADC value,Ktrans,and Ve have the highest application value in evaluating the efficacy of cervical cancer radiochemotherapy,with an Area Under receiver operating characteristic(ROC)Curve(AUC)of 0.947,followed by Ktrans and ADC values of 0.756 and 0.677,respectively;The sensitivity is 0.864,0.727,0.614,and the specificity is 0.893,0.732,and 0.625,respectively.Conclusion The application of DCE-MRI in the evaluation of treatment efficacy in cervical cancer patients undergoing radiochemotherapy has high clinical application value,which can provide personalized reference for further diagnosis and treatment of patients and is worthy of promotion and application.
