摘要

目的:分析钆塞酸二钠磁共振增强成像评价肝细胞癌分化程度的效果.方法:选取2018 年 6 月—2022 年 10 月菏泽市立医院经手术病理确诊的肝细胞癌患者 103 例为研究对象,均进行钆塞酸二钠磁共振增强成像扫描.以临床病理结果为依据,对患者肝细胞癌分化程度进行分期,分析各指标与肝细胞癌分化程度的相关性.结果:103 例患者中,经手术病理证实,Ⅰ级 32 例,Ⅱ级 30 例,Ⅲ级28 例,Ⅳ级 13 例.各分化级别患者在门脉期病灶-竖脊肌信号比(LMR)、过渡期LMR以及相对廓清率、最高强化率对比,差异无统计学意义(P>0.05),而TILMR、动脉期LMR、肝胆期LMR对比,差异有统计学意义(P<0.05).对比增强比(CER)与分化程度的相关系数最大,而相对强度比(RIR)与分化程度的相关系数最小.结论:肝癌分化程度鉴别诊断中,采用钆塞酸二钠磁共振增强成像技术价值较高,可以准确鉴别不同分期肝细胞癌,诊断准确率较高,为临床治疗提供科学的数据支持.

Abstract

Objective To evaluate the differentiation degree of hepatocellular carcinoma by enhanced magnetic resonance imaging(MRI).Methods A total of 103 patients with hepatocellular carcinoma confirmed by surgical pathology from June 2018 to October 2022 were selected as the study subjects,all of whom underwent Gd-EOB-DTPA enhanced MRI.Based on clinical pathological results,stage the differentiation degree of hepatocellular carcinoma in patients and analyze the correlation between various indicators and the differentiation degree of hepatocellular carcinoma.Results Among the 103 patients,confirmed by surgical pathology,there were 32 cases of grade I,30 cases of grade II,28 cases of grade III,and 13 cases of grade IV.There was no statistically significant difference(P>0.05)in the comparison of lesion-to-muscle ratio signal intensity ratio(LMR),transitional LMR,relative clearance rate,and highest enhancement rate among patients of different differentiation levels,while there was a statistically significant difference(P<0.05)in the comparison of TILMR,arterial LMR,and hepatobiliary LMR.The correlation coefficient between contrast enhancement ratio(CER)and differentiation degree is the highest,while the correlation coefficient between relative intensity ratio(RIR)and differentiation degree is the lowest.Conclusion In the differential diagnosis of liver cancer,the Gd-EOB-DTPA enhanced MRI is of high value,which can accurately identify different stages of hepatocellular carcinoma with high diagnostic accuracy,and provide scientific data support for clinical treatment.
