基于超声组学对乳腺癌新辅助化疗淋巴结疗效评估的临床应用

摘要
目的:探讨基于超声组学对乳腺癌新辅助化疗淋巴结疗效评估的临床应用价值.方法:选取 2021 年 7 月—2023 年 7 月桂林市中医医院收治的乳腺癌伴腋窝淋巴结转移的行新辅助化疗 4~8 周的患者 95 例为研究对象.所有患者均实施基于超声组学对乳腺癌新辅助化疗淋巴结疗效评估干预.比较新辅助化疗前后腋下淋巴结大小、数目、纵横比值,统计新辅助化疗前后髓质消失,钙化及存在彩色血流比例,比较新辅助化疗前后淋巴结血流信号分段、弹性成像评分及淋巴结病灶与周边肌肉组织的应变率比值变化.结果:新辅助化疗后腋下淋巴结大小小于新辅助化疗前,数量少于新辅助化疗前,皮质厚度小于新辅助化疗前,差异均有统计学意义(P<0.05);新辅助化疗后存在髓质消失、钙化及存在彩色血流的比例低于新辅助化疗前,差异均有统计学意义(P<0.05);新辅助化疗后淋巴结血流信号分段分级低于新辅助化疗前,弹性成像评分小于新辅助化疗前,ESR值低于新辅助化疗前,差异均有统计学意义(P<0.05).结论:基于超声组学可有效评估乳腺癌新辅助化疗淋巴结的疗效,对提高患者生活质量的一定价值.
Abstract
Objective To investigate the clinical value of the efficacy of neoadjuvant chemotherapy in breast cancer.Methods 95 patients with neoadjuvant chemotherapy for 4-8 weeks admitted to Guilin Municipal Hospital of Traditional Chinese Medicine from July 2021 to July 2023 were included.All enrolled patients performed an ultrasonic omics-based intervention to assess the efficacy of breast cancer neoadjuvant chemotherapy in lymph nodes.We compared the size,number and aspect ratio of axillary lymph nodes before and after neoadjuvant chemotherapy,counted the disappearance of medulla,calcification and the ratio of color blood flow before and after neoadjuvant chemotherapy,and compared the change of lymph node flow signal segmentation,elastography score and the strain rate ratio of lymph node lesions and surrounding muscle tissue before and after neoadjuvant chemotherapy.Results After neoadjuvant chemotherapy,the size and number of axillary lymph nodes were smaller than before,and the cortical thickness was smaller than before,with statistical significance(P<0.05).The proportion of medullary disappearance,calcification,and color blood flow after neoadjuvant chemotherapy was lower than before neoadjuvant chemotherapy,and the differences were statistically significant(P<0.05).The grading of lymph node blood flow signals after neoadjuvant chemotherapy was lower than before neoadjuvant chemotherapy,and the elastography score was lower than before neoadjuvant chemotherapy,The ESR value was lower than before neoadjuvant chemotherapy,and the differences were statistically significant(P<0.05).Conclusion Ultrasonography can effectively evaluate the efficacy of neoadjuvant chemotherapy in lymph nodes of breast cancer,and has a certain value in improving the quality of life of patients.
