摘要
目的:探究全脑CT灌注成像(CTP)参数在大脑中动脉(MCA)狭窄患者预后评估中的应用价值.方法:选择 2020年8月—2022 年 9 月临沂市中心医院收治的 88 例MCA狭窄患者为研究对象,所有患者均行CTP检查,分析数字减影血管造影(DSA)结果、MCA不同狭窄程度以及预后患者CTP检查参数、不同预后MCA狭窄患者健侧CTP检查参数、CTP参数判断MCA狭窄患者预后的价值.结果:MCA不同狭窄程度患者患侧MTT、TTP、DLY均长于健侧,CBV低于健侧,差异有统计学意义(P＜0.05);完全闭塞患者患侧及健侧MTT、TTP、DLY均长于重度及中度狭窄患者,CBV低于重度及中度狭窄患者,差异有统计学意义(P＜0.05).发生脑血管事件患者MTT、TTP、DLY均长于未发生脑血管事件患者,CBV低于未发生脑血管事件患者,差异有统计学意义(P＜0.05).CTP参数(MTT、TTP、DLY、CBV)联合检测在MCA狭窄患者预后评估中的应用价值较高.结论:CTP参数在MCA狭窄患者预后评估中具有较高的临床应用价值,通过对各参数水平的分析,有助于判断患者病情发展情况,及时更换治疗方案,以改善预后.
Abstract
Objective To explore the application value of whole brain CT perfusion imaging(CTP)parameters in the prognosis assessment of patients with middle cerebral artery(MCA)stenosis.Methods A total of 88 patients with MCA stenosis admitted to Linyi Central Hospital from August 2020 to September 2022 were selected.All patients underwent CTP.Digital subtraction angiography(DSA)was the"gold standard"to analyze the prognostic value of CTP parameters in patients with different stenosis degrees.Results Compared with the healthy side,the level of mean transit time(MTT),time to peak(TTP)and delay time(DLY)was higher and the level of cerebral blood volume(CBV)was lower on the affected side with different degree of stenosis(P＜0.05).The levels of MTT,TTP and DLY in patients with complete occlusion were higher than those in severe and moderate stenosis,and CBV was lower than those in severe and moderate stenosis,with statistical difference(P＜0.05).Compared with the control group,the levels of MTT,TTP and DLY were higher in the study group,while the levels of CBV were lower(P＜0.05).The combined detection of CTP parameters has the highest application value in the prognosis assessment of MCA stenosis patients.Conclusion The parameters of CTP have high clinical application value in the prognosis assessment of MCA stenosis patients.Through the analysis of the level of parameters,it is helpful to judge the progression of patients'disease,and it is helpful to change the treatment plan in time to improve the prognosis.
