摘要
目的:分析双源CT检查在冠心病早期诊断中的应用价值及相关敏感指标.方法:选择 2021 年 10 月—2023 年 9 月于汶上县人民医院进行双源CT检查的 199 例疑似冠心病患者为研究对象,均行冠状动脉造影检查,以冠状动脉造影结果为金标准,分析双源CT检查在冠心病早期诊断中的应用价值及相关敏感指标[心外膜脂肪组织(EAT)体积和心周脂肪组织(PAT)体积].结果:冠状动脉造影结果显示,199例疑似冠心病患者中阳性108例、阴性91例.以冠状动脉造影结果为金标准,双源CT检查诊断冠心病的灵敏度为93.52%、特异度为97.80%、准确率为95.48%、阳性预测值为98.06%、阴性预测值为92.71%.冠心病患者EAT和PAT大于非冠心病患者(P<0.05).Logistic回归分析结果显示,CT指标EAT和PAT是冠心病的独立危险因素(P<0.05).受试者工作特征(ROC)曲线分析结果显示,CT指标EAT和PAT及两者联合在冠心病早期诊断中的曲线下面积(AUC)分别为 0.734、0.839、0.867,具有一定的价值,且EAT和PAT联合的应用价值较高.结论:双源CT检查在冠心病早期诊断中的应用价值较高,其中EAT和PAT是其相关敏感指标.
Abstract
Objective To analyze the application value and CT index of dual-source CT in the early diagnosis of coronary heart disease(CHD).Methods A total of 199 patients with suspected CHD who underwent dual-source CT in Wenshang County People's Hospital from October 2021 to September 2023 were selected.All patients were diagnosed by coronary angiography,and coronary angiography was used as the gold standard,to analyze the application value of dual-source CT in the early diagnosis of CHD and CT index[epicardial adipose tissue(EAT)volume and pericardial adipose tissue(PAT)].Results In this study,199 suspected patients with CHD were diagnosed by coronary angiography,of which 108 were positive and 91 were negative.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of dual-source CT were 93.52%,97.80%,95.48%,98.06%and 92.71%respectively.EAT and PAT in CHD group were higher than those in non-CHD group(P<0.05).Logistic regression analysis showed that CT indexes EAT and PAT were associated with coronary heart disease(OR>1,P<0.05).Receiver operating characteristic(ROC)curve was drawn and it was found that the area under the curve(AUC)of EAT and PAT and their combination in the early diagnosis of CHD were 0.734,0.839 and 0.867,respectively,which had certain predictive value,and the combination had high application value.Conclusion Dual-source CT has high application value in the early diagnosis of coronary heart disease,among which EAT and PAT are the relevant CT indexes.
