高频超声联合弹性成像及TI-RADS分级诊断老年甲状腺癌患者颈部淋巴结转移效能分析

摘要
目的:分析高频超声、超声弹性成像、TI-RADS分级联合诊断老年甲状腺癌患者颈部淋巴结转移的价值.方法:选取 2020 年1 月—2024 年 1 月临沂市第四人民医院收治的 48 例老年甲状腺癌疑似颈部淋巴结转移患者为研究对象,所有患者均接受高频超声检查、超声弹性成像检查及TI-RADS分级,以手术病理结果为金标准分析高频超声、超声弹性成像、TI-RADS分级单一及联合诊断颈部淋巴结转移的效能.结果:病理检查结果显示,48 例老年甲状腺癌疑似颈部淋巴结转移患者最终确诊为颈部淋巴结转移(恶性)36 例,非颈部淋巴结转移(良性)12 例.恶性淋巴结长径长于良性、位于颈部Ⅲ区比例高于良性(P＜0.05).以病理结果为金标准,高频超声、超声弹性成像及TI-RADS分级联合诊断老年甲状腺癌患者颈部淋巴结转移的特异度高于TI-RADS分级、灵敏度及准确率高于高频超声(P＜0.05).结论:高频超声、超声弹性成像、TI-RADS分级联合诊断老年甲状腺癌患者颈部淋巴结转移的价值较高,可切实提高诊断准确率.
Abstract
Objective To analyze the application value of TI-RADS,ultrasound elastography and high frequency ultrasound in the diagnosis of cervical lymph node metastasis in senile thyroid cancer.Methods 48 elderly patients with thyroid cancer suspected to have cervical lymph node metastasis admitted to the Fourth People's Hospital of Linyi City from January 2020 to January 2024 were selected.All patients received high-frequency ultrasound examination,elastic imaging examination and TI-RADS classification.The results of surgical pathology were used as the gold standard to analyze the effects of TI-RADS,ultrasound elastography and high-frequency ultrasound in single and combined diagnosis.Results The pathological results showed that 36 cases had cervical lymph node metastasis.The average length and neck area of malignant lymph node metastasis were significantly different from those of benign lymph node metastasis(P<0.05).The specificity,sensitivity and accuracy of the combined diagnosis of the three methods for cervical lymph node metastasis in elderly patients with thyroid cancer were higher than those of high-frequency ultrasound,ultrasound elastography and TI-RADS grading single diagnosis,among which the specificity and TI-RADS grading were significantly different,and the sensitivity and accuracy were significantly different from high-frequency ultrasound(P<0.05).Conclusion The combined application of TI-RADS,ultrasound elastography and high-frequency ultrasound has high diagnostic value for cervical lymph node metastasis in elderly patients with thyroid cancer,and can effectively improve the diagnostic accuracy.
