颈部血管超声诊断脑梗死并颈动脉狭窄价值分析

摘要
目的:分析颈部血管超声检查在脑梗死并颈动脉狭窄中的应用价值.方法:选取 2022 年 1 月—2023 年 7 月寿光市人民医院收治的 300 例疑似脑梗死并颈动脉狭窄患者作为研究对象,所有患者均接受颈部血管超声及数字减影血管造影检查,以数字减影血管造影检查结果为诊断金标准,分析颈部血管超声检查对颈动脉狭窄的诊断效能、不同面积脑梗死患者颈动脉狭窄程度,不同颈部血管超声颈动脉狭窄程度脑梗死患者中膜厚度.结果:颈部血管超声检查诊断颈动脉狭窄的灵敏度、特异度、准确率分别为 98.90%、99.15%、99.00%;随着梗死灶面积的增大,颈动脉狭窄程度增加;随颈部血管超声颈动脉狭窄程度增加,中膜厚度逐渐增厚(P＜0.05).不同颈部血管超声颈动脉狭窄程度脑梗死患者间中膜厚度差异有统计学意义(P＜0.05).结论:颈部血管超声检查对脑梗死并颈动脉狭窄具有较高的诊断效能,且能判断颈动脉狭窄病变严重程度.
Abstract
Objective To analyze the application value of cervical vascular ultrasonography in patients with cerebral infarction and carotid artery stenosis.Methods A total of 300 patients with suspected cerebral infarction complicated with carotid artery stenosis admitted to Shouguang People's Hospital from January 2022 to July 2023 were selected.All patients were required to undergo cervical vascular ultrasonography.The diagnostic efficacy of cervical vascular ultrasonography on diseases was analyzed with digital subtraction angiography(DSA)results as the diagnostic gold standard.To analyze the degree of carotid artery stenosis in patients with different infarcts,and to study the degree of carotid artery stenosis and media thickness of patients with cervical vascular ultrasound.Results The diagnostic sensitivity,specificity and accuracy of cervical vascular ultrasonography were 98.90%,99.15%and 99.00%respectively.With the increase of infarct size,the severity of concurrent carotid artery stenosis will increase.The thickness of the media in patients with grade 4 carotid artery stenosis was higher than that in patients with grade 1,grade 2 and grade 3,P＜0.05.Conclusion Cervical vascular ultrasonography has a high diagnostic efficacy for cerebral infarction complicated with carotid artery stenosis,and can judge the severity of carotid artery stenosis and cerebral infarction in patients.
