Objective To investigate the diagnostic value of color Doppler flow imaging(CDFI)in early diabetes nephropathy(DN)and the analysis of renal artery hemodynamics.Methods A total of 92 patients with suspected early DN admitted to Ningjin County Hospital of Traditional Chinese Medicine from January 2021 to January 2024 were selected for CDFI examination.Renal puncture biopsy or clinical comprehensive judgment was used as the gold standard to analyze the detection and diagnostic value of DN,and Kappa test was used to verify the consistency of CDFI diagnosis of DN with the gold standard.The peak systolic velocity(PSV),end-diastolic velocity(EDV)and resistance index(RI)of renal aorta and interlobar renal artery were compared between DN and non-DN patients.Results Of 92 suspected patients,52 were confirmed by gold standard,of which 51 were detected by CDFI,with a positive rate of 98.08%.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of CDFI were 96.15%,97.50%,96.74%,98.04%and 95.12%respectively.Kappa test showed that the Kappa value of CDFI and gold standard=0.934,which was very consistent.The PSV[(60.24±5.34)cm/s,(27.52±2.33)cm/s]and EDV[(17.15±1.37)cm/s,(9.25±1.14)cm/s]of renal aorta and interlobar renal artery in DN group were lower than those in non-DN group.RI[(0.69±0.12),(0.65±0.11)]were higher than those in the non-DN group(P<0.05).Conclusion CDFI has high value in the early diagnosis of DN.It can not only clarify the morphological characteristics of the kidney,but also detect abnormal renal artery blood flow,which is conducive to early identification of renal lesions and targeted treatment.
