Comparison on the Curative Effect of Median Anterior Knee Approach and Medial-lateral Double-incision Approach on Schatzker Types Ⅴ and Ⅵ Tibial Plateau Fractures
Objective To compared the curative effect of median anterior knee approach and medial-lateral double-incision approach on Schatzker types Ⅴ and Ⅵ tibial plateau fractures. Methods A retrospective analysis was performed on the data of 106 patients with tibial plateau fracture(Schatzker type Ⅴ,Ⅵ)undergoing surgical treatment in the Shangshui County People's Hospital between September 2020 and September 2022.According to different surgical methods, they were divided into median anterior incision group(n=62)and medial-lateral incision group(n=44).The surgical status, recovery of lower limb function, postoperative curative effect and occurrence of postoperative complications were compared between the two groups. Results The operation time, intraoperative blood loss and drainage volume in median anterior incision group were lower than those in medial-lateral incision group, and fracture healing time was longer than that in medial-lateral incision group. In Merchant, scores of activities of daily living and knee stability in median anterior incision group were higher than those in medial-lateral incision group, and scores of pain and range of motion were lower than those in medial-lateral incision group, the difference was statistically significant(P<0.05).Conclusion Compared with medial-lateral knee incision approach, median anterior knee incision approach can effectively shorten operation time and reduce intraoperative blood loss in Schatzker types Ⅴ and Ⅵ TPF.There is no significant difference in postoperative recovery of joint function or the risk of complications between the two methods. However, median anterior knee incision approach has slightly longer fracture healing time.
