The Effect of Laparoscopic Myomectomy on pelvic Floor Function and ovarian Function in Patients with uterine Fibroids
Objective To explore the effects of laparoscopic myomectomy(LM)on pelvic floor function and ovarian function in patients with uterine fibroids.Methods A total of 80 patients with uterine fibroids admitted to Anyang District Hospital in Puyang City from January 2021 to October 2022 were included and divided into two groups by drawing lots.The control group received 40 cases of abdominal myomectomy(AM),while the study group received 40 cases of LM.The pain factors[myocardial cell substance P(SP),prostaglandin E2(PGE2)],stress response indicators[norepinephrine(NE),cortisol(COR)],immune function indicators(CD3+,CD4+,CD8+,CD4+/CD8+),pelvic floor function indicators[pelvic floor impact questionnaire 7(PFIQ-7)]and ovarian function indicators[estradiol(E2),luteinizing hormone(LH),follicle stimulating hormone(FSH)]before and after surgery were compared between the two groups.Results At postoperative 1 day and 3 days,the serum levels of SP,PGE2,NE,and COR in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).On the 1st day after surgery,the functional indicators of CD3+,CD4+,and CD4+/CD8+in both groups decreased compared to before surgery,but the study group was significantly higher than the control group,with statistical significance(P<0.05);The CD8+ functional indicators of the two groups increased compared to preoperative levels,but the study group was significantly lower than the control group,with a statistically significant difference(P<0.05).At 1 month and 3 months after surgery,both groups showed an increase in PFIQ-7 scores,but the study group was significantly lower than the control group,with a statistically significant difference(P<0.05).At 3 months and 6 months after surgery,the serum E2 levels in the study group were higher than those in the control group,while the serum LH and FSH levels were lower than those in the control group,with statistical significance(P<0.05).Conclusion Performing LM on patients with uterine fibroids can effectively reduce the body's pain level,stress response and negative impact on immunity,pelvic floor and ovarian function,the advantages are more significant and have further value for use.
