Diagnostic Value of Serum IL-6,PCT,CRP,and WBC in Early Postoperative Pulmonary Infection in Elderly Patients Undergoing Abdominal Surgery
Objective To investigate the diagnostic value of serum interleukin-6(IL-6),procalcitonin(PCT),C-reactive protein(CRP),and white blood cell count(WBC)in the early diagnosis of postoperative pulmonary infection in elderly patients undergoing abdominal surgery.Methods A total of 80 elderly abdominal surgery patients admitted to Ruyang County People's Hospital from May 2022 to May 2023 were selected as the research subjects,and they were divided into an infection group(n=27)and a non-infection group(n=53)according to the postoperative lung infection status.Compare the levels of serum IL-6,PCT,CRP,and WBC in the two groups before and after surgery at 6 hours,24 hours,and 72 hours;analyze the diagnostic value of combined detection of serum indicators at 24 hours after surgery for early postoperative pulmonary infection in elderly patients undergoing abdominal surgery using ROC analysis;assess the infection severity of patients based on the CPIS score;compare the levels of serum indicators in patients with different degrees of infection at 24 hours and 72 hours after surgery;and analyze the correlation between serum indicators at 24 hours after surgery and CPIS score.Results The levels of IL-6,PCT,CRP and WBC in infected group were higher than those in uninfected group at 6 hours,24 hours and 72 hours after operation,and the differences were statistically significant(P<0.05).The AUC level of serum IL-6,PCT,CRP and WBC at 24 hours after surgery for combined diagnosis of postoperative pulmonary infection in elderly patients with abdominal surgery was 0.882.The levels of IL-6,PCT,CRP and WBC in patients with severe infection were higher than those in patients with mild infection at 24 hours and 72 hours after surgery,with statistical significance(P<0.05).Serum IL-6,PCT,CRP,WBC were positively correlated with CPIS score 24h after infection,with statistical significance(P<0.05).Conclusion Serum IL-6,PCT,CRP and WBC levels are closely related to early postoperative lung infection in patients with abdominal surgery,which is of great significance for clinical diagnosis of early postoperative lung infection.
