Prognostic Significance of CT Examination in Patients with Type B Interwall Aortic Hematoma
Objective To investigate the prognostic significance of CT examination in patients with type B interwall aortic hematoma. Methods The clinical data of 201 patients with type B interaortic hematomas admitted to Henan Chest Hospital from January 2016 to April 2022 were retrospectively analyzed, and the patients were divided into a good prognosis group(179 cases)and a poor prognosis group(22 cases)according to the occurrence of endpoint events.Clinical data and laboratory test indexes of the good prognosis group and the poor prognosis group were compared, analyzed risk factors affecting the prognosis of patients, survival analysis was performed by Kaplan-Meier method. Results The proportion of pericardial effusion, maximum aortic diameter and maximum hematoma thickness in the poor prognosis group were higher than those in the good prognosis group, and the differences were statistically significant(P<0.05).Pericardial effusion, maximum aortic diameter and maximum hematoma thickness were independent risk factors for poor prognosis, and the differences were statistically significant(P<0.05).Log-rank test showed that the survival time of patients with no pericardial effusion, maximum aortic diameter<34 mm and maximum hematoma thickness<8 mm was significantly higher than that of patients with pericardial effusion, maximum aortic diameter≥34 mm and maximum hematoma thickness≥8 mm,and the differences were statistically significant(P<0.05).Conclusion Pericardial effusion, maximum aortic diameter and maximum hematoma thickness can better predict the progression and prognosis of type B aortic interwall hematoma.
