The Efficacy of Ulinastatin Combined with Blood Purification in the Treatment of Elderly Patients with Severe Acute Pancreatitis Complicated with MODS and its Influence on Inflammatory Cytokines
Objective To study the clinical effects of ulinastatin combined with blood purification in elderly patients with severe acute pancreatitis patients complicated by multiple organ dysfunction syndrome(MODS).Methods Seventy-eight elderly patients with severe acute pancreatitis complicated with MODS admitted to Nanyang Central Hospital from June 2022 to December 2022 were randomly divided into two groups.The control group(n=39)received blood purification treatment,while the experimental group(n=39)received ulinastatin combined with blood purification treatment.The total effective rate,inflammatory factor levels[procalcitonin(PCT),C-reactive protein(CRP),Interleukin-8(IL-8),tumor necrosis factors(TNF-α)],organ function[renal function:serum creatinine,glomerular filtration rate(GFR),liver function:alanine aminotransferase(ALT),Alkaline phosphatase(ALP),cardiac function:creatine kinase isoenzyme(CK-MB)],and time of remission of fever,abdominal pain,nausea and vomiting were compared between the two groups.Results The treatment effective rate of the experiment group(94.87%)was higher than that of the control group(79.49%),and the difference was statistically significant(P＜0.05).The remission time of clinical symptoms was shorter,the levels of inflammatory factors and organ functions were similar before treatment in the two groups,after treatment,the levels of inflammatory factors,creatinine,ALT,Alp and CK-MB in both groups were decreased,and the GER value was increased.Compared with the control group,the experimental group was better(P＜0.05).The relief time of clinical symptoms in the experimental group was shorter than that in the control group(P＜0.05).Conclusion Ulinastatin and blood purification can effectively reduce the inflammatory factors and clinical symptoms,improve the total effective treatment rates and organ function for elderly severe acute pancreatitis patients with MODS.
