通窍活血汤治疗痰瘀互结型急性脑梗死的疗效及作用机制分析

摘要
目的 探讨痰瘀互结型急性脑梗死患者辅以通窍活血汤治疗的临床效果.方法 纳入 2019 年 1 月至 2022 年6 月于鹤壁市中医院接受诊治的 120 例急性脑梗死患者,用随机数字表方式将患者分为对照组与观察组,各 60 例.对照组给予急性脑梗死西医对症治疗干预,观察组同时辅以中药方剂通窍活血汤.比较两组患者在病情疗效,治疗前后的脑血流动力学[平均血流速度(Vm)、收缩峰血流速度(Vp)]、神经功能修复[美国国立卫生研究院卒中量表(NIHSS)、血清S100β蛋白、脑源性神经营养因子(BDNF)].结果 治疗后,观察组治疗有效率高于对照组;观察组患者Vm、Vp检测指标高于对照组;观察组NIHSS、血清S100β蛋白低于对照组,BDNF水平高于对照组,差异有统计学意义(P＜0.05).结论 为急性脑梗死患者配合通窍活血汤治疗方案对病情有良好的改善价值,同时可以更好地改善脑血流及神经功能,具备突出的协同效果.
Abstract
Objective To explore the clinical effect of using Tongqiao Huoxue decoction in the treatment of acute cerebral infarction patients with phlegm and blood stasis syndrome.Methods A total of 120 patients with acute cerebral infarction who received treatment at Hebi Traditional Chinese Medicine Hospital from January 2019 to June 2022 were included in the study.The patients were randomly divided into control group and observation group using a random number table,with 60 cases in each group.The control group was treated with symptomatic Western medicine intervention for acute cerebral infarction,while the observation group was supplemented with traditional Chinese medicine formula Tongqiao Huoxue decoction.The therapeutic effects,cerebral hemodynamics[mean blood flow velocity(Vm),peak systolic blood flow velocity(Vp)],neurological function repair[National Institutes of Health Stroke Scale(NIHSS),serum S100β Protein and brain derived neurotrophic factor(BDNF)]were compared between two groups of patients before and after treatment.Results After treatment,the effective rate of treatment in the observation group was higher than that in the control group;The Vm and Vp detection indicators in the observation group were higher than those in the control group;The levels of NIHSS,S100β protein and BDNF in the observation group were significantly lower than those in the control group,with a statistically significant difference(P＜0.05).Conclusion The combination of Tongqiao Huoxue decoction treatment plan with acute cerebral infarction patients has good value in improving the condition,and can better improve cerebral blood flow and neurological function,with outstanding synergistic effects.
