Effect of Maxillary anterior Traction combined with Spiral Arch Expander on the soft and hard Tissues of the maxillofacial Surface of skeletal Class Ⅲ Malocclusion during dental Replacement Period
Objective To treat skeletal class Ⅲ malocclusion(mixed dentition stage)in children with spiral expansion and contraction treatment based on maxillary anterior traction,and to observe the therapeutic effect and its impact on the soft and hard tissues of the patient's teeth and maxillofacial region.Methods A total of 60 children with skeletal class Ⅲ malocclusion during the mixed dentition period treated at Xinyang Central Hospital from January 2021 to December 2022 were divided into two groups using a random number table method.30 cases in the control group were treated with maxillary anterior traction,while 30 cases in the combination group were treated with spiral expanders on the basis of maxillary anterior traction.The clinical efficacy of the two groups was evaluated,and the maxillofacial structure,upper airway gap,and three-dimensional indicators before and after treatment were compared.Results The clinical effective rate of the combination group(86.67%)was higher than that of the control group(63.33%),and the difference was statistically significant(P<0.05).After combined treatment,the indicators related to maxillofacial structure in the combination group were higher than those in the control group,including ANB,MP-FH,ANS-Me,and SN-PP.The protrusion of L1-MP was lower than that in the control group,and the differences were statistically significant(P<0.05).The PNS-R of the combined group after treatment was higher than that of the control group,and the difference was statistically significant(P<0.05).After combined treatment,the three-dimensional indicators of the upper airway including NP area,NP core,NP sag,and NP volume were higher in the combination group compared to the control group,and the differences were statistically significant(P<0.05).Conclusion The combination of maxillary anterior traction and spiral expander has a definite therapeutic effect,which can effectively promote the recovery of maxillofacial structure and upper airway status in children with skeletal Class Ⅲ malocclusion during the mixed dentition period,improve the soft and hard tissue status of the maxillofacial area,and have significant advantages and practical significance.
