目的 探究糖尿病酮症酸中毒使用小剂量胰岛素联合生长因素治疗效果。方法 选择 2020 年 12 月至 2022 年12 月期间郑州大学第五附属医院收治的 94 例糖尿病酮症酸中毒患者,应用随机数表法将其分为对照组(小剂量胰岛素)及观察组(联合生长抑素)各 47 例,比较两组临床指标改善时间、血糖、血钾、血钠、血清淀粉酶、酮体、甘油三酯、炎症因子、应激状态和不良反应发生率的变化情况。结果 观察组临床症状改善时间短于对照组,差异有统计学意义(P<0。05)。观察组血糖、血钠低于对照组、血钾高于对照组,差异有统计学意义(P<0。05)。观察组血清淀粉酶、酮体、甘油三酯水平低于对照组,差异有统计学意义(P<0。05)。观察组白细胞介素-6(IL-6)、C反应蛋白(CRP)、肿瘤坏死因子α(TNF-α)低于对照组,差异有统计学意义(P<0。05)。观察组抗氧化能力(TAC)、氧化氢酶(CAT)、谷胱甘肽过氧化物酶(GSH-Px)水平高于对照组,差异有统计学意义(P<0。05)。观察组不良反应发生率高于对照组,差异有统计学意义(P<0。05)。结论 小剂量胰岛素联合生长抑素治疗糖尿病酮症酸中毒,可快速改善患者临床症状,控制血糖水平,降低炎症水平,提升机体抗氧化能力。
Efficacy and Safety of low-dose Insulin combined with Somatostatin in Patients with Diabetes Ketoacidosis
Objective To explore the therapeutic effect of low-dose insulin combined with somatostatin on diabetes ketoacidosis.Methods From December 2020 to December 2022,94 patients with diabetes ketoacidosis admitted to the Fifth Affiliated Hospital of Zhengzhou University were randomly divided into control group(low-dose insulin)and observation group(combined somatostatin)with 47 cases each.The treatment efficiency,the improvement time of clinical indicators,blood sugar,blood potassium,blood sodium,serum amylase,ketone body,triglycerides,inflammatory factors,stress state and the incidence of adverse reactions of the two groups were compared.Results The improvement time of clinical symptoms in the observation group was shorter than that in the control group,and the difference was statistically significant(P<0.05).The observation group had lower blood glucose and sodium levels than the control group,and higher blood potassium levels than that in the control group,with statistically significant differences(P<0.05).The levels of serum amylase,ketone bodies,and triglycerides in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The levels of interleukin-6(IL-6),C-reactive protein(CRP),tumor necrosis factor α(TNF-α)in the observation group were significantly lower than those in the control group,the difference was statistically significant(P<0.05);The levels of total antioxidant capacity(TAC),catalase(CAT),and glutathione peroxidase(GSH-Px)in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The incidence of adverse reactions in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Low-dose insulin combined with somatostatin in the treatment of diabetes ketoacidosis can rapidly improve the clinical symptoms,control the blood sugar level,reduce the inflammatory level,and enhance the antioxidant capacity of the body.
