Study on the Predictive Value of Sperm DFI and Sperm Morphology for Male Fertility and Assisted Reproductive Technology
Objective To explore the predictive value of sperm DNA fragmentation index(DFI)and sperm morphology for male fertility and assisted reproductive technology.Methods A retrospective analysis of the correlation between the pregnancy outcome of 150 cycles of artificial insemination(IUI)from June 2021 to June 2022 in Kaifeng City Hospital and DFI was conducted,and the biochemical pregnancy rate,early clinical pregnancy rate,and clinical pregnancy rate of different groups(DFI<15%low DFI group and DFI≥15%high DFI group)were observed.Combined with male sperm morphology and lifestyle,the relationship between sperm DFI and sperm morphology,male age,and lifestyle was analyzed.Results In the IUI clinical outcome,the biochemical pregnancy rate(17.71%)and clinical pregnancy rate(15.62%)of the low DFI group were higher than those of the high DFI group(16.67%,12.96%),but the difference was not statistically significant(P>0.05);the early clinical pregnancy rate of the low DFI group IUI(3.12%)was lower than that of the high DFI group(14.81%),and the difference was statistically significant(P<0.05);the sperm vitality,sperm concentration,and normal morphology rate of the low DFI group were higher than those of the high DFI group,and the difference was statistically significant(P<0.05);the sperm DFI of the sperm parameters in the normal reference range was lower than that of the abnormal sperm parameters.The sperm DFI of non-liquefied semen,sperm concentration,vitality,and normal morphology rate lower than the lower limit of the normal reference value is higher than the sperm DFI of liquefied semen higher than the lower limit of the reference,and the difference is statistically significant(P<0.05);there is a significant difference in sperm DFI at different age stages,and there is a statistical significance in the difference in sperm DFI between any two groups in the three groups(P<0.05);there is a significant difference in sperm DFI at different drinking times,and there is no drinking and drinking times>2 times/week,1~2 times/week,>2 times/week group differences are statistically significant(P<0.05);the difference in sperm DFI is statistically significant when comparing the group smoking>20 cigarettes/d with the other three groups(P<0.05);correlation analysis found that sperm DFI is negatively correlated with sperm liquefaction status,sperm vitality,sperm concentration,and normal morphology rate,and is positively correlated with male age and bad lifestyle.Conclusion Sperm DFI has a strong correlation with sperm morphology.Bad lifestyle will increase sperm DFI,which will lead to a decrease in male fertility.It can be seen that by observing the changes in sperm DFI,male fertility can be evaluated,but the changes in sperm DFI are difficult to effectively predict the clinical pregnancy outcome of IUI.Therefore,for males with weak fertility,clinicians do not recommend deciding on assisted reproductive technology programs by detecting sperm DFI.
